摘 要:目的 用表面增强激光解吸离子化飞行时间质谱 (SELDI TOF MS)和蛋白质芯片检测乳腺癌患者血清蛋白质指纹图谱 ,初步探讨筛选候选肿瘤标志物建立的诊断模型在乳腺癌诊断中的临床意义。方法 用SELDI TOF MS技术及配套蛋白质芯片检测 4 9例乳腺癌和 37例非乳腺癌疾病患者的血清蛋白质指纹图谱 ,并运用SPSS10 0软件判别分析处理数据和筛选标志物 ,以建立诊断模型。结果 6个蛋白质峰 [86 11、16 82 7、2 5 711、2 8931、2 5 4 85和 2 4 37质荷比 (m/z) ]组合构建的诊断模型Ⅰ鉴别乳腺癌和非乳腺癌疾病的敏感性为 81 6 % (4 0 / 4 9) ,特异性为 78 4 % (2 9/ 37)。 6个蛋白质峰 (4 4 70、10 85 4、19193、3883、2 0 11和 74 70m/z)组合的诊断模型Ⅱ鉴别Ⅰ期乳腺癌与非乳腺癌疾病的敏感性为 80 0 % (12 / 15 ) ,特异性为 89 2 % (33/ 37)。 5个蛋白质峰 (2 72 6、2 70 14、2 2 4 7、4 4 77和19333m/z)组合的诊断模型Ⅲ鉴别Ⅰ期与Ⅱ~Ⅳ期乳腺癌的敏感性为 91 2 % (31/ 34) ,特异性为93 3% (14 / 15 )。结论 SELDI TOF MS在乳腺癌的诊断尤其是早期诊断、术前分期及候选肿瘤标志物筛选等方面具有一定价值 。Objective To detect the serum proteomic patterns in breast cancer patients by using SELDI-TOF-MS protein chip array technology, screen biomarker candidates, build diagnostic models and evaluate its clinical significance.Methods SELDI-TOF-MS protein chip was used to detect the serum proteomic patterns of 49 patients with breast cancer and 37 patients with non-cancer breast diseases. The diagnostic models were developed and validated by discriminant analysis.Results The model Ⅰcomposed by 6 protein peaks (8611, 16827, 25711, 28931, 25485 and 2437m/z) could do the best in the diagnosis between breast cancer and non-cancer breast diseases.Its sensitivity and specificity were 81.6% (40/49) and 78.4% (29/37). The model Ⅱformed by 6 protein peaks (4470, 10854, 19193, 3883, 2011 and 7470m/z) could do the best in the diagnosis between stage Ⅰ breast cancer and non-cancer breast diseases. Its sensitivity and specificity were 80.0% (12/15) and 89.2% (33/37). Another 5 peaks (2726, 27014, 2247, 4477 and 19333m/z) could build the model Ⅲ to distinguish stageⅠand stage Ⅱ~Ⅳ breast cancer. Its sensitivity and specificity were 91.2% (31/34) and 93.3%(14/15).Conclusion The method showed great potential for the early detection, staging before operation and screening novel and better biomarkers for breast cancer.
