肝细胞癌CT表现特征与VEGF、bFGF及凋亡相关蛋白表达的关系

摘　　要：目的　检测肝细胞癌 (hepatocellularcarcinoma ,HCC)组织中血管内皮生长因子 (vascularendothelialgrowthfactor ,VEGF)、碱性成纤维细胞生长因子 (basicfibroblastgrowthfactor ,bFGF)及凋亡相关蛋白的表达与CT表现特征之间的关系。 方法　对 3 8例共 40个经病理证实且行动、静脉双期CT增强扫描的HCC病灶进行分析 ,用免疫组化SP法检测癌组织中VEGF、bFGF以及fas、bax、bcl -2和bcl -xl的表达情况 ,并结合病理分级将CT各个表现特征与免疫组化的结果进行对照分析。结果　VEGF、bFGF、fas、bax、bcl-2和bcl-xl的阳性表达率分别为 77.5 % ( 3 1/ 40 )、75 % ( 3 0 / 40 )、2 0 % ( 8/ 40 )、2 5 % ( 10 / 40 )、2 7.5 % ( 11/ 40 )和5 0 % ( 2 0 / 40 )。VEGF分别与bFGF、bcl -2之间 ,bcl-2与bcl-xl之间 ,以及bax分别与bcl -2、bFGF之间均具有一定的相关性 (Ρ<0 .0 5 )。CT显示的病灶大小与bcl-xl的表达具有一定的关系 (P <0 .0 5 ) ;病灶边缘清晰与否与VEGF的表达和病理分级具有一定的相关性 (Ρ <0 .0 5 ) ;瘤内液化坏死与病理分级有关 (Ρ <0 .0 1) ;强化类型和侵袭转移性则与肿瘤的大小有一定的关系 (Ρ<0 .0 5 )。结论　HCC的某些CT表现特征可在一定程度上反映VEGF和bcl-xl的表达 。Objective To detect the relationship between CT features and the expressions of vascular endothelial growth factor (VEGF), basic fibroblast growth factor (bFGF) and some apoptosis relative proteins including fas, bax, bcl-2 and bcl-xl in hepatocellular carcinoma (HCC).Methods Thirty-eight cases (40 lesions) with HCC verified by histopathology and examined with enhancement dual-phase CT scanning (arterial phase and portal vein phase) were included in this study. The expressions of VEGF, bFGF, fas, bax, bcl-2 and bcl-xl were detected with immunohistochemical SP method. The CT features were compared with the immunohistochemical and pathological results.Results The positive rate of VEGF,bFGF,fas,bax,bcl-2 and bcl-xl was 77.5%(31/40),75%(30/40),20%(8/40),25%(10/40),27.5%(11/40) and 50%(20/40) in HCC respectively.The correlations between VEGF and bFGF or bcl-2,between bax and bcl-2 or bFGF,and between bcl-2 and bcl-xl were found in this study (Ρ<0.05).As for the relationship of CT features and other results,there were close correcations between bcl-xl expression and the diameter of lesion on image (Ρ<0.05),between the patterns of lesion edge on CT images and VEGF expression or pathological grade (Ρ<0.05),between the intratumoral deliquescence and necrosis on the images and pathological grade (Ρ<0.01),and between the enhancement patterns or the high infiltration and severity (intrahepatic daughter foci,tumor-emboli in portal veins,lymphatic metastasis) shown on CT images and the diameters of the tumors (Ρ<0.05).Conclusion Some CT features of HCC can reflect the expressions of VEGF and bcl-xl in certain degree,and the CT features also are influenced by some other factors.
