胸部孤立性转移瘤X线表现与临床特点

摘　　要：目的 :探讨胸部转移瘤的X线特点 ,提高对胸部孤立性转移瘤的认识和诊断水平。方法 :回顾性分析了近 10年来我院经病理或临床观察证实的 4 8例胸部孤立性转移瘤X线胸片 ,对其特点进行分析。结果 :本组 4 8例孤立性转移瘤中 4 0例肿瘤X线表现为圆形、边缘光滑、密度均匀阴影。病变边缘呈浅分叶状 4例 ,3例边缘有毛刺、分叶。 4 8例中发现转移瘤最早者为查出原发肿瘤前 2年 ,最迟者是原发肿瘤术后 17年 ,4例在证实原发肿瘤的同时发现肺内病灶 ,其余均在半年至 5年内不等。发生部位包括肺内 4 4例 ,纵隔内 1例 ,胸膜 3例。结论 :胸部孤立性转移瘤缺乏临床症状 ,有时难与原发性肿瘤鉴别 ,其特点为病变多呈圆形、密度均匀、边缘光滑 ,右肺多于左肺 ,好发于肺外围。恶性肿瘤术后应常规定期胸部X线检查。Objective:To study the radiography feature and clinical characteristics of the solitary thorax metastasis, and to evaluate the value of radiography in solitary thorax metastasis.Methods:It was selected that patient with malignant tumors proved by pathology in our hospital in recent10 years. Solitary node was detected in 48 cases by following thoracic X-ray examinations. X-ray topography was performed in parts of cases.Results:The earliest time at which the thoracic metastasis was found two years before the finding of the primary tumor.The latest one was 17 years after the surgery of the primary malignant tumors. In 3cases,thoracic metastasis was found at same with the primary tumors. Others were found in 3 or 5 years after the primary tumors was proved. The locations of the metastases were pulmonary of 44 cases, mediastinum of 1 cases, pleura of 3 cases.28 cases of the metastases could be ablated. After interventional therapy, 6 cases healed. New metastases were found in the following examination in some cases, and parts of them gave up further therapy.Conclusion:It was lack of special clinical symptom in the solitary thorax metastasis and it was difficult to distinguish the primary and the metastasis. Most of thorax metastases occurred in the right lung. The curative effect of the operation in metastasis was not different with the primary. It is important to carry routine thoracic examination for the postoperative malignant tumors. Thoracic radiography is the most convenient and simple method.
