支气管动脉栓塞配合放射治疗中晚期肺癌的临床应用研究

摘　　要：目的探讨支气管动脉栓塞配合放射治疗中晚期肺癌的临床应用研究,为提高中晚期肺癌治疗价值.方法36例肺癌,进行支气管动脉大剂量抗癌药物灌注及栓塞,每次治疗间隔时间为20～30d.在介入治疗20 d后可以行胸片及CT定位,采用二野或三野体外放射治疗,照射野超出肿瘤局部2 cm.治疗方案为:2000CGr,放疗后休息2周.结果该组36例中,临床症状及实验室检查;咳嗽,痰中带血好转或消失,食欲增加,痰脱落细胞学转阴(22/36);肿瘤大小变化:肿瘤缩小＜25.0%6例,＜25.0%～50.0%10例,＜50.0%以上18例;生存期:生存6个月以上20人,占治疗人数的50.0%;生存12个月以上12人,占33.3%;生存20个月以上4人,占11.1%,无1例严重并发症发生.结论支气管动脉灌注栓塞及放疗.是治疗肺癌的一种有效率较高,毒副作用较小的最安全方法.Objective:To evaluate the clinical application and study of bronchial arterial embolization cooperated with radiotherapy treat intermediate and advanced cancer of lung for increasing therapeutic value.Methods:36case with lung cancer were treated by bronchial artery infusion with high dose anticancer drugs and embolization,there is20to30days interval to the next therapy,to fix position with CT and sternum20days after interventional treatment,adopted limit2or limit3external radiotherapy,range of irradiation beyond reach of tumor part2cm.therapeutic schedule is2000CGr,have a rest for two weeks.Results:By observing clinical symptoms and result of laboratory ex-amination,in36cases.There has been an improvement or disappearance in cough,bloody sputum,appetite getting better,sputum negative conversion rate is elevated by exfoliative cell examination of sputum.tumor size:of those tu-mor masses of6cases were shrank less than25.0%,10cases were shrank between25.0%~50.0%,18cases were shrank above50.0%.Survival periods:the six months,12months,20months survival rate for the patients were50.0%(20cases),33.3%(12cases),11.1%(4cases)respectively,without any complication.Conclusions:Bronchial artery infusion embolization and radiotherapy is a method of safety,high efficiency,tiny side effect.
