原发乳腺非霍奇金淋巴瘤的临床分析

摘　　要：目的　分析原发乳腺非霍奇金淋巴瘤的临床特点、诊断、治疗及预后。方法　对 18例原发于乳腺的非霍奇金淋巴瘤进行分析 ,其中Ⅰ期 11例 ( 61.1% ) ,Ⅱ期 7例 ( 3 8.9% )。 1例 ( 5 .6% )为未分型NHL ,17例 ( 94.4% )为B细胞来源。 15例( 83 .3 % )先行手术治疗 ,术后 3例行放疗 ,7例行化疗 ,5例行放疗、化疗综合治疗。未手术的 3例均行放疗、化疗的综合治疗。结果　中位随访时间为 89个月 ,6例 ( 3 3 .3 % )出现局部复发或远处播散。总 3、5年生存率分别为 87.5 %和 71.5 % ,3、5年无进展生存率分别为 75 .6%和 68.1%。结论　对原发乳腺非霍奇金淋巴瘤 ,采用合理的放疗、化疗综合治疗可获得较好疗效 ;Objective To analyze the clinical characteristics,diagnosis,treatment and prognosis of primary Non-Hodgkin′s lymphoma of breast(PNHLB).Methods 18 patients diagnosed as PNHLB were reviewed.11 patients presented with stage I disease(61.1%)and 7 with stage Ⅱ disease(38.9%).One patient was unclassified Non-Hodgkin′s lymphoma(5.6%) and another seventeen(94.4.1%) had B cell type.Fifteen patients(83.3%) received surgery first,of which,three patients received radiotherapy,seven received chemotherapy and five received combined radiochemotherapy after surgery.Another three patients received combined radiochemotherapy alone.Results The median follow-up for the whole group was 89 months.Six patients(33.3%) had local relapse or distant metastasis.The overall survival rates were 87.5% at 3 years and 71.5% at 5 years.The disease free survival rates were 75.6% at 3 years and 68.1% at 5 years.Conclusion Combined radiochemotherapy is the optimal treatment for PLB.Anthracycline based regimen is recommended.
