奥氮平治疗精神分裂症阳性症状的对照研究

摘　　要：目的 以氟哌啶醇为对照,探讨新型的抗精神病药奥氮平对有明显阳性症状的精神分裂症的疗效和安全性。方法对符合CCMD-3中精神分裂症的诊断标准,有明显的阳性症状(PANSS量表总分≥60分,阳性症状评分≥12分)患者随机分为奥氮平组和氟哌啶醇组各34例,于治疗前和治疗后1、2、4、6周末进行PANSS评分和TESS评价副反应。结果 两组内的PANSS量表评分从疗后1周起至6周末均较疗前有显著下降,具有统计学意义。对两组间5次PANSS评分分别进行两两比较,结果发现在治疗的前3周内无显著差异(P>0.05),而从第4、5、6周时PANSS评分减分率奥氮平组明显大于氟哌啶醉组,具有显著性差异(P<0.01)。结论奥氮平(商品名再普乐)在治疗精神分裂症阳性症状和药物安全性方面优于氟哌啶醇,并进一步提示奥氮平可作为急性发作精神分裂症患者第一线的首选药物。Objective To study the effectiveness and safety of a new type of neuroleptics, Olanzapine. in the treatment of schizophrenic patients with significant positive symptoms by using haloperidol as a control. Methods Patients met the diagnostic criteria of schizophrenia of CCMD- Ⅲ with significant positive symptoms(PANSS score≥60,positive symptoms score≥ 12)were randomly divided into Olanzapine Group and haloperidol Group, 34 cases in each group. Positive symptoms and side effect were evaluated before treatment and at the end of week 1、2、4 and 6 after treatment using PANSS and TESS. Results Within-group comparison of PANSS scores showed that scores from the end of week 1 to week 6 after treatment reduced statistically more significant than the score before treatment in each group. Paired comparison of between-group score among 5 times of PANSS assessment showed there were no significant difference within the first 3 weeks after treatment (P>0. 05). But from the end of week 4,5 and 6,the PANSS score reduced rate in Olanzapine group were statistically more significant than those in aloperidol group(P<0. 01). Conclusion Olanzapine(Zyprexa) was better and safer than haloperidol in treating positive symptoms of schizophrenia. This further suggested that Olanzapine could be a first choice and first line drug in treating acute attack of schizophrenia.
