大肠恶性肿瘤合并皮肌炎治疗分析

摘　　要：目的　大肠恶性肿瘤合并皮肌炎 7例治疗特点。方法　对 7例大肠恶性肿瘤合并皮肌炎病例进行分析。结果　 5例症状明显改善 ,1例部分改善 ,1例死亡。结论　大肠恶性肿瘤合并皮肌炎有其特点 :( 1)皮肌炎易并发阻塞性肺炎 ,肺不张 ,肺功能不全等并发症。 ( 2 )长期应用大剂量肾上腺皮质激素可导致术后刀口愈合延迟或不愈合 ,吻合口瘘。 ( 3)可伴有各种内脏损伤 ,出现脏器功能不全。 ( 4 )预后较差 ,老年人比年轻人预后更差。 ( 5 )用药禁忌 :应用青霉素类药物可诱发结缔组织病。因此应采取包括手术在内的综合治疗。充分的术前准备 ,合理应用肾上腺皮质激素控制皮肌炎症状和及时的术中 .术后观察处理 。Objective: To analyze the therapies of the colorectal neoplasms accompanied by dermatomyositis; Methods: Seven cases of colorectal neoplasms accompanied by dermatomyositis were analyzed Results: Five cases were completely controlled, one case was partly controlled and one was dead Conclusion: Colorectal neoplasms accompanied by dermatomyositis have some characteristics, such as obstructive lung diseases, unhealed wound, dysfunction of some viscera,bad prognosis, inapplicability of penicillins, and so on As a result, we should apply combined modalitytherapy Enough preparation for operation, reasonable steroid therapy to control the dermatomyositis and prompt observation and dealing before and during the operation play an important role in the treatment for the colorectal neoplasms accompanied by dermatomyositis
