术中放疗结合增敏剂顺铂治疗食管癌21例初步报告

摘　　要：1991年5月～12月,对食管癌病人施行术中放疗21例。食管切除之前,肿瘤和肿瘤周围组织给予15～20GY的大剂量照射。照射之前,用10～20mg顺铂静脉点滴和肿瘤周围注射。手术后再对纵隔进行40～60GY的外照射。所有病人术后恢复都很顺利,无并发症发生。本文对术中放疗的临床意义、适应证、放射增敏剂顺铂的使用,放射剂量、技巧和可能出现的并发症进行了讨论。Intraoperative radiation therapy (IORT) was done in 21 patients with esophageal cancer from May 1991 to December 1991. A radiation dose of 15-20 GY was delivered before esophagectomy to the tumor and peritumoral tissue. Intravenous and peritumoral injection of 10-20mg cisplatin was performed 4 hours before operation and prior to IORT respectively. External irradiation of 40-60GY(8- 16Mev photon beam)was delivered to the mediastinum after recovery from surgery. The postoperative course was uneventful and no complications developed in all patients. The clinical significance, technique, indications, doses and possible complications of IORT and the use of radiosensitizing agent cisplatin were discussed.
