阿霉素与庆大霉素治疗三叉神经痛的早期疗效对比研究

摘　　要：目的 :比较不同浓度阿霉素及庆大霉素用于三叉神经痛的治疗效果。方法 :5 0例三叉神经痛患者随机分为 3组 ,分别用 0 .2 %、0 .5 %阿霉素及庆大霉素行外周神经阻滞。观察治疗前、治疗后 1天、3天、1周、2周、1月、2月、3月时的疼痛缓解情况及治疗过程中出现的不良反应。结果 :0 .2 %阿霉素组除治疗后第 1天外其余各时间点数字评分 (NRS)均较治疗前降低 ,而其它两组治疗后各时间点NRS均明显下降。 0 .5 %阿霉素组在治疗后 1天、2周、1月、2月、3月时的NRS值明显低于 0 .2 %阿霉素组 ,治疗后 1月、2月时明显低于庆大霉素组。 0 .5 %阿霉素组有效率明显高于0 .2 %阿霉素及庆大霉素组。结论 :0 .5 %阿霉素对三叉神经痛的治疗效果较好 。Objective: To observe the therapeutic effects and adverse effects of adriamycin at different concentration and gentamicin on trigeminal neuralgia. Methods: Fifty patients with trigeminal neuralgia were divided into three groups randomly. They received nerve block peripherally with gentamicin or adriamycin at 0.2% or 0.5%, respectively. The parameters included pain intensity before and 1d, 3d, 1w, 2w, 1m, 2m and 3m after treatment, pain relief rate and dosage of carbamazepine. The adverse effects were also evaluated. Results: The pain intensity in 0.5% adriamycin group and gentamicin group decreased significantly at each time point ( P <0.01). In 0.2% adriamycin group, obvious decrease were observed at all except the first day after treatment ( P <0.05). In 0.5% adriamycin group, the value of numeric rating scale (NRS) was significantly less than that in 0.2% adriamycin group at 1d, 2w, 1m, 2m and 3m after treatment, and less than that in gentamicin group at 1m, 2m. The effective rate in 0.5%, 0.2% adriamycin and gentamicin groups were 72.2%, 25% and 31.3%, respectively. Conclusion: 0.5% adriamycin is more suitable for trigeminal neuralgia than 0.5% adriamycin and gentamicin.
