摘 要:目的探讨应用支气管哮喘患者基础肺功能指标预测其气道高反应性(AHR)程度(PC35sGaw)的可行性。方法对51例支气管哮喘发作期、113例支气管哮喘缓解期行基础肺功能检查及支气管激发试验。结果支气管哮喘发作期患者PC35sGaw与FEV1实测值/预计值%、sGaw%、MMEF%、PEF%等肺功能指标相关性较好,特别与FEV1实测值/预计值%相关系数高达0702。回归分析显示,FEV1实测值/预计值%的变化对PC35sGaw的影响最大;支气管哮喘缓解期患者PC35sGaw与其它肺功能参数相关性均较支气管哮喘发作期患者差,回归分析显示只有sGaw%对PC35sGaw有影响。结论在无条件行支气管激发试验的单位,支气管哮喘患发作期者可根据基础FEV1实测值/预计值%推测患者AHR程度,有一定临床实用价值。支气管哮喘缓解期患者基础肺功能指标对AHR程度的预测作用较差。ObjectiveTo investigate the feasiblity of forecasting the degree of airway hyperresponsiveness (AHR) by basic pulmonary function tests in patients with asthma.Methods 164 patients with asthma in which 51 cases were in acute exacerbation and 113 cases in remission were included in the study.All patients undertook basic pulmonary function tests followed by methacholine bronchoprovocation tests.Results ①The FEV 1 %,sGaw%,MMEF%,PEF% showed a good correlation with PC 35 sGaw in the patients with exacerbation in which FEV 1 % showed a highest correlation coefficient of 0 702 with and most strong impact on PC 35 sGaw.②All basic pulmonary function test parameters except sGaw% showed poor coorelation with PC 35 sGaw in the cases of remission.sGaw% showed a weak impact on PC 35 sGaw by regression analysis.Conclusion FEV 1 % is a relatively feasible parameter to predict the degree of airway hyperresponsiveness (AHR) in exacerbation of asthma which is a alternative for the medical facilities where the AHR test is inavailable,whereas the predicting value in remission is of little significance.
