摘　　要：目的探讨经颞枕下入路切除岩斜坡脑膜瘤的显微手术方法。方法总结17例岩斜坡区脑膜瘤的临床表现、影像学特征及术中处理。结果肿瘤全切16例,次全切除1例。手术并发症发生率为29.4%,1例(5.8%)因多脏器功能衰竭死亡。结论经颞枕下入路切除岩斜坡脑膜瘤效果良好,对岩骨尖进行处理可提高术野显露程度;选择合适的操作间隙有利于肿瘤的切除及减小手术损伤。Objective To discuss the microsurgical technique in dealing with the petroclival meningiomas by modified subtemporal-occipital approach. Method Seventeen cases of petroclival meningiomas were retrospectively reviewed for its clinical presentation, imaging characteristic as well as surgical technique. Results Total resection was achieved in 16 cases, and subtotal resection in 10 cases. Rate of postoperative complication was 29.4%, one patient died. Conclusion The modified subtemporal-occipital approach could produce better result when dealing with the petroclival meningiomas. Removal of the petrosal apex could improve the operative exposure, choosing proper operating space will be helpful for the tumor resection and reduce the postoperative complication.
