睾丸原发性恶性淋巴瘤6例报道并文献复习

摘　　要：目的　探讨原发性睾丸恶性淋巴瘤的临床及病理形态特征与预后的关系。方法　对 6例原发性睾丸恶性淋巴瘤进行组织形态学和免疫组化染色观察 ,依据WHO新分类进行分型 ,并结合文献对其临床病理特点及随访资料进行分析。结果　患者年龄 4 6～ 82岁。病程 2周～ 1年 ,除 2例伴有疼痛外均为单侧睾丸无痛性肿大。 6例均为弥漫性大B细胞性淋巴瘤 (DLBCL) ,其中 5例为中心母细胞性 ,1例为中心母细胞中心细胞混合性。瘤细胞LCA、CD4 5RA和CD2 0均 (+)。随访 4例 ,2例分别于首诊后 5、10个月出现鼻咽部、胸膜累及 ;2例分别为 13和 18个月无瘤生存。结论　睾丸原发性淋巴瘤以弥漫性大B细胞性非霍奇金淋巴瘤为主。老年人多见。预后与临床分期及组织学分型关系密切。Objective To observe the clinical and histopathologic characteristics of primary malignant lymphoma of the testis. Methods Six cases of primary malignant lymphoma in the testis were observed with HE and immunohistochemical stains. The clinical, histologic features, immunohistochemical results of the tumor and their follow-up data were also reviewed. The lymphomas were classified by the WHO criteria. Results The age of patients were 46 to 82 years (mean 66.7) and all of the patients were frist seen with an enlarging testicular mass of 2 weeks to 1 year. The mass was painless in 4 patients and painful in 2. Immunohistochemical staining showed that six cases were positive for LCA, CD45RA and CD20 . All of the patients were diagnosed as diffuse large B cell lymphomas (DLBCL) at the Stage I. Five were centroblastic types, and one was diffuse mixed centroblastic/centrocytic type. Follow-up results were obtained in 4 patients, in which two patients were still alive without diseasefor 13 to 18 months after diagnosis; The other two patients had nasopharynx and pleura after a disease-free interval of 5 and 10 months respectively. Conclusions Most primary testicular lymphomas are diffuse large B cell NHL. It occurs frequently in elderly men. The initial stage of disease and morphological findings are associated with prognosis.
