脑梗死患者血清细胞因子白细胞介素6水平变化与神经功能评分及其他相关因素的关系

摘　　要：目的:脑梗死的不同时期,监测血清细胞因子白细胞介素6(interleukin-6,IL-6)的变化,并分析与之相关的因素。方法:上海第二医科大学附属第九人民医院神经内科2002-07/2003-02收治脑梗死患者28例,发病24h内入院,在脑梗死发病的不同时期测定血清IL-6水平并与对照组21例比较,检验该因子与不同因素之间的相关性。结果:根据非参数统计结果,脑梗死组急性期IL-6水平比对照组明显升高(P<0.05),亚急性期及恢复期水平与对照组无差异,不同梗死体积组之间无差异。脑梗死急性期IL-6水平与神经功能评分、白细胞计数有相关性(r=0.43425,0.6173,P<0.05),与梗死体积、梗死部位、血小板计数、纤维蛋白原含量无相关性(r=0.2587,0.1225,0.18353,-0.0279,P>0.05)。结论:脑梗死患者急性期血清IL-6水平明显升高,与神经功能评分、白细胞计数相关。AIM:To measure the change of serum cytokine interleukin 6 (IL 6) levels in different phases of cerebral infarction and analyze the related factors. METHODS:From July 2002 to February 2003,twenty eight inpatients with cerebral infarction hospitalized within 24 hours after attack were selected from the Department of Neurology,the Ninth People's Hospital of Shanghai Second Medical University.The levels of serum IL 6 at different phases of cerebral infarction were measured in the 28 patients with cerebral infarction and 21 controls for comparison.Correlation analysis was done between IL 6 levels and different factors. RESULTS:After non parametric statistics,IL 6 levels in the acute phases of cerebral infarction significantly increased as compared with the control group(P< 0.05).But there were no differences in subacute and recovery phases between the patient and control groups, and no differences in the subgroups of different cerebral infarct volume.IL 6 levels in the acute phase of cerebral infarct was correlated with score of European Stroke Scale(ESS) and white blood cell(WBC) count(r=0.434 25,0.617 3,P< 0.05),but not with cerebral infarct volume, cerebral infarct type,platelet count and fibrinogen content(r=0.258 7,0.122 5,0.183 53,-0.027 9,P >0.05). CONCLUSION:Serum IL 6 levels increased significantly in acute phase of cerebral infarction, which is correlated with ESS score and WBC count.
