不明原因青年脑卒中患者的血浆蛋白Z水平与年龄、体质量指数及纤维蛋白原的关系

摘　　要：目的:为了寻找青年脑卒中患者的共同危险因素,探讨不明原因青年脑梗死和脑出血中血浆蛋白Z(plasmaproteinZ,PZ)水平的变化及与年龄、体质量指数及纤维蛋白原的关系。方法:采用病例对照研究,共纳入天津渤海石油医院神经内科收治的33例不明原因青年脑梗死患者、39例不明原因青年脑出血患者,同时纳入78例体检者作为对照,以上对象抽取静脉血,采用酶联免疫吸附双抗体夹心法测定其血浆PZ水平。结果:脑梗死组PZ水平为(2.85±0.69)mg/L,显著高于对照组(2.07±0.80)mg/L,差异有非常显著性意义(q=6.1977,P<0.01);脑出血组PZ水平显著低于对照组(P<0.01);脑出血组PZ水平为(1.22±0.53)mg/L,显著低于对照组(q=4.6448,P<0.01)。脑梗死组PZ水平与年龄、体质量指数、纤维蛋白原水平正相关(r分别为0.505,0.436,0.620,P<0.05)。结论:PZ水平的升高/降低可能与青年不明原因脑卒中有关,但进一步的结论还有待研究。AIM:To investigate the relation of plasma protein Z(PZ) with age,body mass index(BMI) and fibrinogen in the youth with cerebral infarction and cerebral hemorrhage of unknown origin for seeking the common risk factor of young stroke patients. METHODS:It was a case control trial.Totally 33 young patients with cerebral infarction and 39 with cerebral hemorrhage whose etiological factors are unknown were selected from the Department of Neurology,Bohai Oil Hospital of Tianjin.And 78 subjects for health examination were enrolled as controls.Venous blood was extracted as blood sample.Level of PZ was measured with enzyme linked immunosorbent assay. RESULTS:PZ level was(2.85±0.69) mg/L in the cerebral infarction group,significantly higher than the control group[(2.07±0.80) mg/L],and there was a significant difference between the two groups(q=6.197 7,P < 0.01).However,the level of PZ in the cerebral hemorrhage group was(1.22±0.53)mg/L,significantly lower than that in the control group(q =4.644 8,P< 0.01).In the cerebral infarction group,the level of PZ had a positive relation with age,BMI,fibrinogen level(r=0.505,0.436,0.620,P< 0.05). CONCLUSION:The increase of decrease of PZ level may relate to the stroke of unknown origin in the young, but it need further study.
