摘  要:背景:紫外线照射充氧自血回输疗法(ultravioletbloodirradiationandoxygenation,UBIO)促进了急性脑梗死患者神经功能的修复,但对其作用机制仍有待进一步研究。目的:探讨UBIO对急性脑梗死患者红细胞膜泵功能的影响。设计:随机对照的实验研究。单位:襄樊市中心医院神经内科。对象:2000-01/2001-12襄樊市中心医院神经内科住院的急性脑梗死患者58例。将58例急性脑梗死患者随机分成2组,UBIO治疗组和常规治疗组。方法:在治疗前、治疗1次后、治疗3次后、疗程结束后(第14天)动态观察两组脑梗死患者红细胞膜K+-Na+ATPase和Ca2+-Mg2+ATPase活力的变化。主要观察指标:①两组患者红细胞膜K+-Na+ATPase和Ca2+-Mg2+AT-Pase活力动态变化。②神经功能评分。结果:治疗前UBIO治疗组和对照组红细胞膜K+-Na+ATPase活力为63.61±49.77和65.43±39.54,治疗1次后UBIO治疗组和对照组红细胞膜K+-Na+ATPase活力为113.61±69.31,69.52±42.75,两组比较差异有非常显著性意义(P<0.01),治疗3次后,仍然明显高于治疗前,两组比较差异有显著性意义(P<0.05),在疗程结束后K+-Na+ATPase活力绝对值仍然比治疗前高,两组比较差异没有显著性意义(P>0.05)。UBIO治疗组患者的红细胞膜Ca2+-Mg2+ATPase活力在治疗第1次后即明显增加。BACKGROUND:Ultraviolet blood irradiation and oxygenation(UBIO) promotes nerve functional recovery in patients with acute brain infarction,but the mechanism underlying this effect remains unclear. OBJECTIVE:To observe the effect of UBIO on the function of red blood cell(RBC) membrane pump in patients with acute brain infarction. DESIGN:A randomized and controlled study. SETTINGS:Department of Neurology,Central Hospital of Xiangfan. PARTICIPANTS:From January 2000 to December 2001,58 patients with acute brain infarction admitted in the Department of Neurology,Central Hospital of Xiangfan, were recruited and divided into UBIO treatment group and the routine treatment group. METHODS:The dynamic changes of K+ Na+ATPase and Ca2+ Mg2+ATPase activity on the RBC membrane were observed in the both groups of patients before,after the first and third treatments, and after the completion of the treatment course(the 14th day) repsectively. MAIN OUTCOME MEASURES:The dynamic changes in the activity of K+ Na+ATPase and Ca2+ Mg2+ATPase on the RBC membrane were observed in both groups,and the patients' nerve function was scored. RESULTS:The activity of K+ Na+ATPase of the RBC membrane in the UBIO treatment group and the control group was 63.61±49.77 and 65.43±39.54 respectively before the commencement of the treatment.After the first treatment K+ Na+ATPase activity were 113.61±69.31 and 69.52±42.75 respectively,showing significant difference between the two groups(P< 0.01). The activity remained significantly higher than the pre treatment levels and significant difference between the two groups was maintained(P< 0.05).After the completion of the treatment course,K+ Na+ATPase activity was still higher than that of the pre treatment level but the two groups were statistically comparable at this point(P >0.05).In the UBIO treatment group,Ca2+ Mg2+ATPase activity on the RBC membrane significantly increased after the first treatment and remained higher than the pre treatment level throughout the treatment.The activi
