摘 要:目的探讨乙型肝炎病毒(HBV)C基因启动子(CP)和前C区基因变异及HBV基因型与肝硬化的关系。方法通过DNA扩增、基因序列分析检测44例慢性乙型肝炎(CH)、29例肝硬化(LC)患者的血清HBVS基因序列确定其基因型,测定HBVCP和前C区序列确定其变异状况。结果LC患者CP双变异(nt1762A→T和1764G→A)发生率(72.4%)显著高于CH(45.5%),P<0.05;LC患者C型检出率(69.0%)显著高于CH(43.2%),P<0.05;C型HBV感染者的CP双变异发生率(69.2%)显著高于B型感染者(44.1%),P<0.05。结论CP双变异与C基因型密切相关,并且导致病情向肝硬化发展。Objective To study the relationship between mutations in the core promoter(CP) and the precore gene of hepatitis B virus(HBV),hepatitis B genotype and the severity of liver cirrhosis.Metheds S region,CP and precore gene were sequenced directly from sera of 44 patients with chronic hepatitis B and 29 patients with liver cirrhosis after amplication by the polymerase chain reaction.Results CP mutations were observed in 21 of 44 patients with chronic hepatitis B(45.5%)and 22 of 29 patients with liver cirrhosis(72.4%).There was significant difference in the prevalence of CP mutations between patients with chronic hepatitis B and patients with liver cirrhosis. Genotypes B and C occurred in 34(46.6%)and 39(53.4%) of samples. Patients with liver cirrhosis had a higher prevalence of Genotypes C than patients with chronic hepatitis B(69.0%vs 43.2%,P<0.05).Genotypes C had a higher prevalence of T1762/A1764 mutation in CP than genotype B(69.2% vs 44.1%,P<0.05).Conclusion Genotype C is significantly associated with T1762/A1764 mutation in CP,and both of them are closely correlated with the develepment of liver cirrhosis.
