黄芪注射液加等容血液稀释疗法治疗老年脑梗死血瘀证的临床研究

摘　　要：目的 :观察黄芪注射液和等容血液稀释疗法对脑梗死血瘀证的治疗作用。方法 :选择 6 4例住院及门诊脑梗死患者 ,同时符合血瘀证诊断标准 ,其中男 4 3例 ,女 2 1例 ;年龄 6 3～ 76岁 ,平均 ( 6 9.6± 4 .4 )岁。随机分常规治疗组和等容血液稀释疗法加黄芪注射液治疗组 ,观察两组治疗前后血液流变学指标的变化 ,并于治疗3周后进行疗效评定。结果 :黄芪注射液加等容血液稀释疗法治疗组临床疗效较常规治疗组好 ,临床总有效率为 93.75 %比 87.5 0 % ,有显著性差异 ( P<0 .0 5 ) ;治疗后全血黏度、血细胞比容及纤维蛋白原等指标均有显著的改善 ( P均 <0 .0 5 )。结论 :对血瘀证的老年脑梗死患者采用等容血液稀释疗法加益气中药黄芪注射液治疗 ,具有较好的降低血黏度、促进血流、减轻临床症状、减轻致残率、提高脑梗死临床疗效的作用。Objective: To observe the therapeutic effects of astragalus injection and isometric hemo dilution therapy in treating cerebral infarction with blood stasis syndrome(血瘀证) in elderly patients. Methods : Sixtyfour inpatients and outpatients(43 were male and 21 were female) of cerebral infarction with blood stasis syndrome aged from 63 to 76 years old and average in (69 6±4 4)years old were selected. The patients were divided randomly into two groups: treating by routine therapy(routine therapy group, n=32 ) or by astragalus injection and isometric hemodilution therapy(treated group, n =32). The changes of hemorheology in these patients were observed before and after treatment, and the curative effects was compared between the treated group and the routine therapy group at the end of 3 weeks. Results: The clinical curative effects was superior in the treated group to that in the routine therapy group, the total effective rate in the treated group and the routine therapy group was 93 75% and 87 50% ( P <0 05). Observation on hemorheology parameters showed that whole blood viscosity, erythrocyte specific volume and fibrinogen in treated group after treatment were superior to that in the routine therapy group or before the treatment. Conclusion : The research suggests that it is more effective in treating elderly patients of cerebral infarction with blood stasis syndrome by astragalus injection and isometric hemodilution therapy than by routine therapy. It has the effect of decreasing the whole blood viscosity, accelerating blood stream, alleviating clinical symptoms and reducing the rate of resulting in crippledom.
