军人创伤后应激障碍患者关联性负变与失配性负波的变异

摘　　要：目的 :探讨军人创伤后应激障碍 (PTSD)患者关联性负变 (CNV)及失配性负波 (MMN)的变异 .方法 :应用美国NicoletBravo脑诱发电位仪 ,采用光和声成对刺激、反应时间以及听觉靶 非靶刺激序列技术 ,检测 6 6例PTSD和 36名正常人 (NC)的CNV及MMN .结果 :(1)CNV :M1潜伏期PTSD组 [(5 0 3 4± 139 2 )ms]长于NC组 [(42 0 1± 12 3 6 )ms;P <0 0 1],M2 波幅PTSD组 [(18 9± 7 9) μV]高于NC组 [(11 7± 5 8) μV ;P <0 0 1],指令信号后负变化 (PINV)的出现率PTSD组 (39% )高于NC组 (3% ;P <0 0 1) .(2 )MMN :潜伏期PTSD组 [(2 14 2± 2 5 7)ms]长于NC组[(193 7± 2 2 6 )ms;P <0 0 1],波幅PTSD组 [(8 5± 1 8)μV]高于NC组 [(5 4± 2 2 ) μV ;P <0 0 1].结论 :CNV与MMN的变异特点可作为PTSD辅助诊断的脑电生理学标志 .AIM: To investigate the variations of contingent negative variations (CNV) and mismatch negative (MMN) in patients with post-traumatic stress disorder (PTSD). METHODS: Sixty-six cases of patients with PTSD and 36 normal controls (NC) were involved.The CNV and MMN were recorded with Nicolet Bravo Instrument.RESULTS: Compared with NC group, PTSD group showed delayed M 1 latency and increased M 2 amplitude (P<0.01). The post imperative negative variation (PINV) emergence rate of CNV in PTSD group was significant different from that in NC group (P<0.01). Compared with NC group,PTSD group showed further prolonged MMN latency and higher increased MMN amplitude (P<0.01). CONCLUSION: The characteristics of CNV and MMN can served as a physiological index in diagnosing of PTSD.
