摘　　要：目的　探讨儿茶酚胺氧位甲基转移酶 (COMT)基因多态性与自杀未遂的相关性 ,并了解影响COMTVal/Val型自杀未遂者的相关因素。方法　从山东省 7个市、县级综合医院中收集2 0 0 3年一年内自杀未遂者共 2 2 0例 ,分别调查其一般信息和抽取 1ml肘静脉血 ;采用聚合酶链反应 (PCR)和限制性片段长度多态性 (RFLP)分析方法 ,检测COMT基因多态性。采用病例 -病例研究分析COMTVal/Val型与其他因素的相关性。结果　自杀未遂者中女性∶男性 =1.2 3∶1,年龄均集中于 2 0岁到 4 9岁 (70 .1% ) ,多集中小学及以下 (6 0 .9% ) ,睡眠与以前有变化 (占 6 0 .0 % ) ,多为抑郁 (6 9.5 % )和多有情感冲突 (81.0 % )。自杀未遂者中COMT基因型以Val/Met为主(5 4 .5 % ) ,Met/Met仅占 0 .3%。只发现自杀未遂者中精神障碍与COMTVal/Val型具有相关关系(χ2 =6 .110 ,P =0 .0 13)。结论　自杀未遂者中女性与年轻人、文化程度低、具有睡眠问题、抑郁和情感冲突所占比例较高 ;自杀未遂者中存在COMT基因多态性与精神障碍的相互作用。Objective To understand the association between gene polymorphism of catechol-o-methyltransferase (COMT) and attempted suicide, and to find out the correlation factors related to attempted suicide with COMT Val/Val. Methods 220 cases were collected from seven polyclinics at city or county level in Shandong province. The general information was investigated and 1ml blood from elbow vein was also took out from each suicide attempters. PCR-RFLP was used to analyze gene polymorphism of COMT. The case-case study was adopted to analyze the relativity between COMT Val/Val genotype and the other factors. Results The ratio of female to male was 1.23∶1, the age was mainly in 20～49 years old (70.1%), the education level was mainly on high school level (60.9%), the quality of sleep was mainly not well (60%), most of them had depression ( 69.5%) and affective conflicts ( 81.0%). The most of the COMT genotype of attempted suicide was Val/Met ( 54.5%), the least was Met/Met, about 0.3%. The association between mental disorders and COMT Val/Val in suicide attempters was found (χ2=6.110, P=0.013). Conclusions There were more female, youth, low education level, poor quality of sleep, depression and affective conflicts among suicide attempters. And there was interaction between mental disorders and COMT Val/Val in the suicide attempters.
