人类免疫缺陷病毒感染者泪膜功能的研究

摘　　要：目的 探讨人类免疫缺陷病毒(HIV)感染者的泪膜功能。 方法 前瞻性研究34例HIV／AIDS患者,对照组为40例HIV阴性者。分别行眼干症状询问、泪膜破裂时间测定(TBUT)、角膜荧光素染色和基础泪液分泌试验(SIt)。结果 病例组TBUT和SIt结果及TBUT和SIt异常和角膜荧光素染色的比率与对照组比较,差异均无显著性意义(P>0.05)。病例组出现眼干症状的比率明显低于对照组(P<0.05)。C组与A、B组比较,C组SIt结果较低(P<0.05),角膜染色评分较高(P<0.05),SIt结果异常者较多(P<0.05)。不同眼底病变组比较,SIt和TBUT结果无显著性差异(p>0.05)。结论 HIV感染者发生泪膜功能异常的比率并不高于正常人群。HIV感染者较不容易出现干眼症状。重症AIDS患者是发生干眼症的易患人群。Objective To investigate an association of tear film function with the severity of the human immunodeficiency virus (HIV) infection and acquired immune deficiency syndrome (AIDS). Methods Thirty-four patients and 40 age-and sex-matched control subjects were studied prospectively. Patients were classified into three groups according to CD4 + T lymphocyte counts in blood. The CD4 + lymphocyte count for group A,B,C was 200,199-51 and 50/mm3 respectively. Tear film function was evaluated by dry eye syndrome,tear break-up time (TBUT) ,corneal fluorescein staining and Schirmer I test (SIt). Results The difference of TBUT and SIt results between patients and controls was not significant (P > 0. 05 ). The rate of TBUT ( < 10s ) , SIt (≤5mm) and corneal fluorescein staining in patient group were similar to those in control group (P >0. 05). Symptom of dry eye was significantly less in patient group than in control group (P < 0. 05). Corneal fluorescein staining score was higher in Group C than that in Group A and B (P < 0. 05) , while Group C had a higher rate of low SIt (P < 0. 05). The TBUT and SIt results were similar among different fundus abnormality groups. Conclusion HIV-infected patients have a similar abnormal rate of tear film and a less occurring rate of dry eye in comparison with control individuals. Severe AIDS patients are more susceptible to dry eye.
