HRT-II在正常眼压性青光眼诊断中的应用

摘　　要：目的　探讨海德堡视网膜断层扫描仪 II(HRT II)在正常眼压性青光眼 (normaltensionglaucoma ,NTG)诊断中的意义。方法　对 2 4例 4 8眼NTG患者进行HRT II检测和自动视野计检查 ,统计分析HRT II检测的敏感性、与视野的对应关系、参数和病变分区特点。结果　 2 4例 4 8眼中视野异常 4 1眼 ,HRT II检测异常 4 1眼 ;视野异常和HRT II异常对应为 91.6 7% ;单眼视野异常组 7例有对侧 2眼视野正常而HRT II检测异常 ,对侧 5眼视野正常而HRT II检测也正常。视盘参数为 :视盘面积 (1.5 6 7± 0 .349)mm2 ,视杯面积为 (1.4 4 7± 0 .2 6 3)mm2 ,盘沿容积为 (0 .16 3± 0 .14 7)mm3 ,盘沿面积为 (0 .6 82± 0 .390 )mm2 ,C/D为 0 .738±0 134,平均视杯深度 (0 .32 7± 0 .12 0 )mm ,平均视网膜纤维厚度为 (0 .193± 0 . 16 1)mm。颞下象限盘沿面积异常占6 9 77% (30 /43) ,颞上为 6 0 .4 7% (2 6 /43) ,鼻下为 76 .74 %(33/43) ,鼻上为 6 0 .4 7% (2 6 /43) ,颞侧为 4 8.84 % (2 1/43) ,鼻侧为 6 0 .4 7% (2 6 /43)。结论　 (1)HRT II检测是NTG诊断的方法之一 ,敏感性良好 ;(2 )HRT II检测和视野改变有良好的对应关系 ,而NTG患者HRT II异常可能早于视野改变 。Objective To investigate the diagnostic value o f HRT-II for normal tension glaucoma(NTG). Methods Twenty-four cases (48 eyes) of NTG were detected by HRT-I I and auto visual fields.The results were analysized among the sensitivity ,corr esponding ratio,and the characters of different disk section. Results Each of the abnormal ratio in visual fields detection a nd HRT-II is 85.42%(41/48).The corresponding ratio between the visual fields de tection and HRT-II is 91.67%(44/48).In the group of single eye abnormal in visu a l field ,there were 2 eyes which were normal in visual field,but abnormal in HR T -II, and 5 eyes were normal in both visual field and HRT-II.The mean value of di sk area (DA) is(1.567±0.349)mm2,the cup area (CA) (1.447±0.263) mm2,rim volu me(RV)(0.163±0.147) mm3,rim area(RA)(0.682±0.390) mm2,C/D 0.738±0.13 4,mean cup depth(MCD)(0.327±0.120)mm,mean retinal nerve layer(MRNFL)(0.193 ±0. 161)mm.The abnormal ratio in tempoal inferior section is 69.77%(30/43),while others are 60.47%(26/43) in temporal superior,76.74%(33/43) in nasal infe r ior,60.47%(26/43) in nasal superior,48.84%(21/43) in temporal,60.47%(26/ 43)in nasal.Conclusion (1)HRT-II is one of the potent diagnosis means fo r NTG with high sensitivity;(2)The results of HRT-II are related to the resu lts of visual field ,and HRT can probably be applied to NTG early diagnosis for its abnormal changes earlier than the visual field.
