经额入路手术治疗大型垂体腺瘤87例报告

摘　　要：目的:总结探讨经额入路治疗大型垂体腺瘤的临床效果和手术指征。方法:对87例手术病例术后随访6个月至5年,回顾性分析手术前后的临床效果。结果:20例全部切除、67例次全切除病例中,3例死亡,8例术后复发。结论:对于大型垂体腺瘤应采用经额入路手术治疗,其切除率高,效果可靠,同时要求在末后接受常规放射治疗。Objective: To assess the clinical effect and operation indication of transfrental surgery of the patients with large pituitary adenomas. Method; Transfrontal operations were performed in 87 cases with large adenomas. All patients were followed up from 6 months to 5 years after surgery. Retrospective analysis was made according to the clinical manifestation of pre and post - operation. Results:Total subsidence of the tumors in 20 cases and remission in 67 cases were achieved. 3 cases died after operation. Recurrence rate after the surgery was 9.2%. Conclusion: The clinical results has shown that transfrental approach is a safer and more effective method for large pituitary adenomas. In order to prevent the recurrence of adenomas, radiotherapy should be recommended after the operation.
