摘  要:目的 创建一种慢性肾盂肾炎 (CPN)大鼠模型。方法 分别用 10 8/ml(A组 )、10 6/ml(B组 )浓度致病性大肠杆菌 (E .coliO111B4)株膀胱内注射 ,并短暂结扎左侧输尿管 ,观察 7、15、30、6 0、90d肾脏形态学变化 ,肾组织及膀胱尿细菌培养 ,以及 30、6 0、90d尿N 乙酰 β D 氨基葡萄糖苷酶 (NAG) /尿肌酐、血肌酐 (Scr)、尿素氮 (BUN)、2 4h尿蛋白定量的变化。结果 随着感染性炎症的修复 ,左肾外观有皮质瘢痕及相关肾乳头收缩和肾盏的扩张 ,镜下慢性肾间质性肾炎改变和纤维化 ,以A组显著 ;A、B两组NAG/尿肌酐在第 90日明显升高 ;细胞培养阳性结果主要在左肾和尿 ;其余项目与C组无差异。结论 短暂结扎单侧输尿管和膀胱内接种一定剂量E .coliO111B4能够制备典型的CPN大鼠模型 ,以 10 8/ml浓度为适宜。Objective To build a rat model of chronic pyelonephritis(CPN).Methods Experimental CPN was created in rat by injecting 0.5ml of 10 8/ml(group A) and 10 6/ml (group B) E.coli O 111 B 4 into urinary bladder via the urethra with temporary left ureter’s ligation.Pathological changes of kidneys and bacterial culture of urine were observed at days 7,15,30,60 and 90.The detection of urinsry N-acetyl-beta-D-glucosamidase (NAG) by NAG/urinary creatinine (Ucr),24h urinary protein,serum creatinine(Scr),and blood urea nitrogen (BUN) were made at day 30,60 and 90.Results Pathological changes in accordance with CPN were observed in the ligated kidney at 90d in group A.The levels of urinary NAG/Ucr should a marked increase in group A and B compared with group C (normal group) at 90d.The results of Scr,BUN,and 24h urinary protein had no difference in three groups.Positive bacterial culture was obtained mainly in the ligated kidney and urine.Conclusions Combining injecting certain dose of E.coli O 111 B 4 into the urinary bladder with temproary unilateral ureter ligation is a good method to produce rat model of CPN,and 10 8/ml concentration is more suitable for making this model than 10 6/ml.
