胃癌腹水临床治疗体会

摘　　要：目的　比较绿慕安辅助腹腔化疗与单纯腹腔化疗对胃癌术后癌性腹水的近期临床疗效。方法　纳入胃癌术后癌性腹水患者腹腔免疫化疗组 2 0例 ,腹腔化疗组 2 0例。腹腔免疫化疗组给予 5 Fu +MMC +绿慕安。腹腔化疗组给予 5 Fu +MMC化疗 ,B超测定患者治疗前后腹水消退率 ,结束后评价肿瘤疗效 ,并在治疗前后分别检测患者免疫功能。结果　在临床疗效评价中 ,腹腔免疫化疗组有效率 (CR +PR)为 6 5 .0 % ,腹腔化疗组为 30 .0 % (P <0 .0 5 )。腹腔免疫化疗组治疗后C3 、C4、CD4/CD8比值 ,NK细胞活性和IL 2水平均高于腹腔化疗组 (P <0 0 5 )。有 3例出现不良反应。结论　绿慕安能明显提高胃癌患者的免疫功能 ,与化疗药物合用可提高疗效。Object To observe the effects of the pseudomonas aeruginosa with mannose sensitive hemagglutination pili (PA-MSHA) vaccine in adjuvant treatment of postoperative gastric cancer. Methods 20 cases of postoperative gastric cancer ascites were treated with intra-peritoneal injection of cisplatin plus PA-MSHA. The levels of ascites were determined by B ultrasonograph before and after the treatment. Immunne function was measured before and after the treatment. Results The short term effective rate of intra-peritoneal injection of cisplatin plus PA-MSHA on postoperative ascites was 65.0%. After treatment ,the C 3,C 4,CD 4/CD 8,NKcell function and IL-2 level in the cisplatin plus PA-MSHA group were higher than those in the cisplatin group respectively(P<0.05). Conclusion The therapy of intra-peritoneal injection of cisplatin plus PA-MSHA was a simple, safe and effective method of treatment for postoperative gastric cancer ascites.
