成人角回的断层解剖学研究

摘　　要：目的　为临床MRI图像精确定位大脑角回的微小占位性病变 ,以及脑功能的影像学立体定位的研究提供断层解剖学依据。方法　选用成人的头颅标本 15例 ,随机分为 3组 ,每组 5例 ,分别做横断面、矢状面和冠状面的MRI扫描后 ,再依据MRI扫描的层次 ,获得与MRI图像一致的断层标本。把标本与相应的MRI图像做对照观察、统计 ,以确定角回在横断面、矢状面和冠状面上的定位。结果与结论　在横断面、矢状面和冠状面 3个方向上描述了角回的断面形态、位置和毗邻关系。 (1)在冠状面上 :①经窦汇的断面 :顶内沟明显纵行 ,枕颞内、外侧回的出现 ,是角回出现的标志。②经胼胝体压部的断面 :胼胝体压部和侧脑室三角区的出现 ,是角回消失的层面。 (2 )在横断面上 :①经中央旁小叶的断面 :扣带回和顶枕沟的出现 ,伴有扣带沟缘支的消失 ,是角回和缘上回出现的标志。②经室间孔的断面 :在切及颞叶结构时 ,大脑外侧沟由“一”变为“人”字形时 ,是角回消失的标志。 (3)在矢状面上 :在外侧第一个层面上角回就已经出现 ,壳核和侧脑室三角区的出现是角回消失的标志。Objective To study the localization of the angular gyrus and provide sectional anatomical data for MRI imaging diagnosis.Methods Fifteen adult cadaveric human brains without correlating diseases were randomly divided into 3 groups and were respectively scanned with MRI in horizontal plane,sagittal plane and coronal plane.After 7 days frozen in -40℃,the brains were sliced in correspondence with MRI scan.Results and Conclusion The cross-sectional shape,location and neighbour of the angular gyrus were described in horizontal,sagittal and coronal plane.1.In coronal plane:on the section through the later horn of lateral ventricles,intraparietal sulcus just line lengthways,the appearance of the intraparietal sulcus and the later horn of lateral ventricles is important signal of the appearance of the angular gyrus and precuneus.On the coronal section through corpus callosum,the corpus callosum and lateral ventricle is the section of the disappearing of the anglular gyrus.2.In horizontal plane:on the section through the paracentral lobule,when the angular gyrus and supramarginal gyrus appeared,the simultaneity of the cingulated gyrus and parietooccipital sulcus appeared,the cingulated sulcus and the marginal ramus disappeared.On the section through the interventricular foramen,the signal of the disappearing of the angular gyrus was the shape of the lateral sulcus changed into 'Y'and the structure of the temporal lobe can be seen.3.In sagittal plane:angular gyrus appeared on the first cortical section.The appearance of putamina and lateral ventricle is an important signal of the disappearing of angular gyrus.
