摘 要:目的:评定优秀长跑运动员和普通大学生直立位运动负荷递增时左室舒张期血流动力学的变化。方法:采用多普勒超声心动图分别记录8名耐力训练者和10名普通学生递增负荷运动下舒张期血流频谱,并对图像进行分析处理。结果:随着运动强度的增加优秀运动员每次左室血液充盈量呈递增趋势,由安静状态下(88.67±4.45)mL增至200W时(140.43±6.7)mL,而普通大学生在125W时增至最大,安静状态下(73.86±4.92)mL增至125W时(97.69±4.08)mL。两组左室血液充盈在每一级负荷时优秀运动员显著大于普通大学生(P<0.05);优秀运动员和普通大学生二尖瓣瓣口血流速度的最大值、平均值、峰值流速加速度和舒张期左室和左房跨瓣压差最大值、平均值均增加,而两组左室射血时间、舒张期充盈时间均随着运动强度的增加而呈递减趋势,在每一级负荷时优秀运动员左室射血时间、舒张期充盈时间显著大于普通大学生(P<0.05)。结论:随着运动强度的增加,优秀运动员每次左室血液充盈量呈递增趋势;而普通大学生每次左室血液充盈量在运动初期增加,在运动负荷125W时犤心率为:(124.0±8.0)次/min犦增至最大,后随着强度的增加基本上维持在一个平台趋势。AIM:To assess the hemodynamic changes of diastolic filling of left ventricle during progressive upright cycle exercise in university students and male elite distance runners. <METHODS:The images in dynamics of left ventricular diastolic filling were recorded during progressive exercise with Doppler echocardiography in 8 male distance runners(study group) and 10 normal young university students(control group), and then the images were analyzed later. <RESULTS:The mitral flow volume per beat in the study group was increased from(88.67± 4.45) mL at rest to(140.43± 6.7) mL at 200 W work loads, while it increased from(73.86± 4.92) mL at rest to the maximal value(97.69± 4.08) mL at 125 W in the control group.Compared with the control group,the left ventricular diastolic filling is higher during each progress in the study group(P< 0.05);With the exercise intensity increasing,the peak and mean mitral flow velocity,the acceleration of the mitral flow and the peak and mean transmitral pressure gradient increased gradually,while the left ventricular ejection time(LVET) and diastolic filling time decreased gradually in both groups.The LVET and diastolic filling time during each progress in the study group were significantly longer than those in the control group(P< 0.05). <CONCLUSION:With the increase of exercise intensity,the left ventricular filling is increased gradually in the elite distance runners,while the that in the university students is increased at the early period of exercise,and reaches its peak value at the work load of 125 W [heart rates:(124.0 ± 8.0) beats per minute],and then maintains at a platform tendency with the increasing intensity of exercise.
