个旧矿工肺癌与其他地区非矿工肺癌中SP-B表达的比较研究

摘　　要：目的 :比较云南个旧地区矿工与其他地区非矿工肺癌组织中表面活性蛋白B (SP -B)表达的差异 ,探讨Ⅱ型肺泡上皮增生与个旧地区肺癌矿粉沉着及纤维化的相关性 .方法 :用免疫组化S -P法检测了5 0例个旧矿工肺癌、 4 0例其他地区非矿工肺癌组织及 17例正常肺组织中SP -B的表达 ,并用图像分析仪对SP -B阳性细胞数及纤维化面积作定量分析 .结果 :个旧矿工肺癌癌周Ⅱ型肺泡上皮细胞数比其他地区非矿工肺癌和正常肺组织都高 (P <0 0 1) ,个旧矿工肺癌矿粉沉着及纤维化明显高于其他地区非矿工组 ,肺泡Ⅱ型上皮细胞增生与肺癌组织的纤维化程度呈正相关 (γs=0 96 1) .结论 :Ⅱ型肺泡上皮增生与个旧矿工肺癌明显的纤维化有关 ,SP -B阳性细胞见于增生和异常增生的Ⅱ型肺泡上皮 ,并与肺鳞癌有移行过度 ,提示部分肺鳞癌有可能起源于Ⅱ型肺泡上皮 .Objective: In order to explore whether hyperplasia of alveolar epithelial cell type Ⅱwas correlated with the deposited mineral dust and the fibrosis of lung cancer in Gejiu miners ,the different expressions of SP-B were investigated in lung cancer specimens of Gejiu miners ,and non-miners in other regions. Methods: SP-B from 50 specimens of Gejiu miners lung cancer ,40 specimens of non- miners lung cancer and 17 specimens of normal lung tissues were detected by the method of immunohistochemistry S-P . The results were quantitatively analyzed using image analysis system. Results: The number of type Ⅱalveolar epithelial adjacent carcinoma in Gejiu miners was much more than that in non-miners of other regions and normal lung tissue(P<0.01). The rate of the deposited mineral dust and lung fibrosis in Gejiu miners was significantly higher than that of non-miners .The proliferating type Ⅱalveolar epithelial cell positively correlated with extent of lung fibrosis ( r s=0.961). Conclusion: Hyperplasia of type Ⅱalveolar epithelial cell may be related to remarkable lung fibrosis of Gejiu miners. The expression of SP-B in hyperplasia and atypical hyperplasia of type Ⅱalveolar epithelial cell, and transition form from them to squamous cell carcinoma was observed. It suggests that some squamous cell carcinoma of lung probably originates from the type Ⅱalveolar epithelial cell.
