摘 要:目的 :评价硬性透氧性角膜接触镜 (RGP)对青少年近视的矫治作用。方法 :5 4例 (10 8眼 ) 15岁以下近视及散光的青少年配戴硬性透氧性角膜接触镜 (RGP) ,同时将 5 4例(10 8眼 )配戴框架眼镜的青少年作为对照 ,三年内每 6个月复查其眼的屈光度、角膜曲率半径、角膜地形图及眼轴长度。结果 :三年后的近视屈光度平均增加RGP组为 (0 .4 9± 0 .76 )D ,框架眼镜组为 (1.5 5± 0 .6 3)D ,二者差异有非常显著性 (P<0 .0 0 1) ;角膜平均变平RGP组为 (0 .38± 0 .34)D ,框架眼镜组为 (0 .14± 0 .36 )D ,二者差异有非常显著性 (P <0 .0 0 1) ;眼轴平均增长RGP组为 (0 .38± 0 .4 3)mm ,框架眼镜组为 (0 .70± 0 .5 3)mm ,二者差异有非常显著性 (P <0 .0 1)。二组角膜表面规则指数 (SRI)及角膜表面不对称指数 (SAI)差异无显收稿日期 :2 0 0 4-0 4-0 8;修回日期 :2 0 0 4-0 9-2 5基金项目 :上海市黄浦区科学技术委员会科研基金资助项目 ;黄浦区科委攻关课题 (HGG -17)。作者简介 :钱进 (1963 -) ,女 ,上海人 ,主任医师 ,研究方向 :白内障及视光学。E -mail:jinjin_0 0 7@citiz.net著性 (P >0 .0 5 )。结论 :RGP可能由于较好地矫正了散光 ,在一定程度上使角膜变平且阻止眼轴增长 ,从而起到阻止近视快速发展的作用。Objective:To study the effect rigid gas permeable(RGP) contact lenses in controlling the progression of myopia in adolescents.Methods:Fifty four myopic children with astigmatism(108 eyes) were selected to wear RGPs,and another 54 myopic children were selected as controls by wearing spectacles. The study was done over 3 years with follow up visits every 6 months. The evaluation included refraction,corneal curvature radius,corneal topography and axial length.Results:This study showed that the mean increase in myopia was (0.49±0.76)D in the RGP group,and (1.55±0.63)D in the control group. There was a significant difference between the 2 groups(P<0.001). The mean corneal flattening in the RGP group was (0.38±0.34)D,and (0.34±0.36)D in the control group. There was a significant difference between the 2 groups(P<0.001). The mean increase in axial length was (0.38±0.43)mm in the RGP group and (0.70±0.53)mm in the control group. There was a significant difference between the 2 groups(P<0.01). There was no significant difference in SRI or SAI(P>0.05).Conclusion:RGPs had a significant effect in controlling rapid myopia progression by flattening the cornea and slowing the increase in axial length.
