摘  要:目的探讨大脑星形细胞瘤的表观扩散系数(ADC值)与质子波谱(1HMRS)主要化合物浓度的相关性。方法经病理诊断的星形细胞瘤23例,其中男11例,女12例。先常规MR扫描,后依次作MR扩散成像(DWI)和单体素1HMRS。计算肿瘤兴趣区MRS主要化合物NAA、Cho、Cr浓度并分别与DWI之ADC值比较。结果瘤体的ADC值为(1.06±0.33)×10-9m2/s,ADC值与Cho浓度之间存在显著负相关(r=-0.66,P<0.001),ADC值与NAA、Cr间无明显相关(r分别为0.26、0.26,P>0.05)。结论DWI与MRS相互结合,有助于在活体上对脑肿瘤的病理微观状态进行判断,对指导外科手术与组织定位活检、评价肿瘤放化疗的疗效具有潜在价值。Objective To investigate the relationship between ADC value and main metabolites level measured by proton MR spectroscopy in cerebral astrocytomas. Methods Twenty-three patients (11 men and 12 women) with pathologically proven cerebral astrocytoma underwent the routine MR imaging, the diffusion-weighted imaging (DWI) and single voxel H-1 MR spectroscopy respectively. The levels of dominant peak of NAA, Cho and Cr, and the ADC values in spectroscopic voxel of interest located within the assumed tumor area were measured and compared. Results A strong correlation (r = -0.66, < 0.001) was found between the ADC value P (1.06±0.33×10-9m2/s)and level of choline.There was, however,no significant correlation between the ADC value and level of NAA or Cr (r =0.26,P > 0.05). Conclusion MRI in combination of DWI and MR spectroscopy may be of potential use in characterizing tumor histopathologic state, guiding surgery or stereotactic biopsy and monitoring chemo-or radiotherapy response.
