^(131)I-肿瘤细胞核人鼠嵌合单抗脑胶质瘤瘤内放免治疗研究

摘　　要：目的　研究13 1I chTNT(13 1I 肿瘤细胞核人鼠嵌合单抗 )通过局部化疗囊注入胶质瘤瘤腔内 ,行瘤内放免治疗。方法　选择全部经手术病理证实的脑胶质瘤患者 5 6例 ,其中胶质母细胞瘤 3 4例 ,星形细胞瘤 2 2例。第 1次手术者 2 0例 ,余为复发性手术。术中瘤腔内置入化疗囊 ,手术后 7d囊内注入13 1I chTNT 1.1× 10 3 Bq ,15d后重复注入 1次。 结果　于末次注药后 2个月复查CT或MRI ,随访期 6个月～ 2年 2个月 ,完全缓解 2 1例 (3 7.5 % ) ,部分缓解 2 4例 (4 2 .8% ) ,临床症状减轻 ,病情稳定 7例 (12 .5 % ) ,病情恶化 4例 (7.1% )。结论　单克隆抗体与13 1I交联后靶向于肿瘤细胞核 ,和瘤体特异性结合 ,并将其荷载的放射性核素输送到肿瘤细胞内 ,由内向外摧毁肿瘤。临床疗效满意 ,优于其他类型的瘤内近距离放疗。Objective To explore the clinical effectiveness of intratumoral radioimmunotherapy for brain gliomas by radioiodinated monoclonal antibody ( 131 I chTNT) via chemotherapy reservoir.Methods Fifty six patients with gliomas pathologically were enrolled,including 34 cases of glioblastomas and 22 cases of astrocytomas.Among these patients subjet to surgery,20 were primary tumors,and the remaining were recurrent tumors.During the operation the chemotherapy reservoir was placed in the resection cavity at the time of tumor debulking.Seven days later,1.1×10 3 Bq 131 I chTNT was injected into the residual tumor via chemotherapy reservoir.The treatment was repeated on the postoperative day 15 20.All the patients were reexamined with CT scan or MRI after 2 months and followed up for 6 months to 2 years.Results Twenty one cases (37.5%) obtained complete remission,24 (42.8%) partial remission.In 7 cases (12.5%), the clinical menifestations were alleviated and the state of the illness was stable,and only 4 cases (7.1%) were deteriorated.Conclusion After monoclonal antibody conjugated with 131 I,the combined agent can d target nucleus of the tumor cells,then the radioiodine is transported into tumor cells and destroyed the cell.Intratumoral radiotherapy by 131 I chTNT performs efficiently,and demonstrates a significant virtue compared with other interstitial brachytherapies for malignant brain tumors.
