5例杂合性急性白血病临床特点  

CLINICAL FEATURES OF 5 CASES OF HYBRID ACUTE LEUKEMIA

摘　　要：利用单克隆抗体及免疫酶法(ABC法)检测92例次急性白血病免疫表型,发现5例HAL,其中AML伴CD_(19)^+或CD_(10)^+者4例,其细胞表达CD_(10),CD_(19),CD_(20),CD_(13),CD_(14),HIM_4,HIM_5,CD_(37),CD_(416)。4例均有肝或脾或淋巴结肿大,POX阳性率较低,治疗反应差,预后不好。ALL伴CD_(13)^+者1例,细胞表达CD_(10),CD_(19),CD_(13),无肝、脾、淋巴结肿大,经治疗虽获CR但缓解期短,很快复发。HAL较一般急性白血病治疗反应不好,预后差。检测白血病细胞表面抗原有助于HAL的诊断。5 cases of hybrid acute leukemia were identified among the 92 immunophenotype of patients with acute leukemia assayed by using ABC method and McAbs. 4 cases of them were AML with CD_(19)^+ or CD_(10)^+, the surface markers were CD_(10)^+、CD_9^+、 CD_(20)^+、CD_(13)^+、CD_(14)^+、HIM_4^+、HIM_5^+、and CD_(41b)^+. All 4 cases had splenomegaly or hematomegaly or lymphadenovarix. Histochemical examination was done and we found that the POX postive rate was lower than general AML. ALL were reflactory and had poor porgnosis. 1 cases of them was ALL with CD_(13)^+ and without splenomegaly, hemotomegaly or lymphadenovarix. She achieved complete remission(CR), but the period of CR were shorter than general ALL. Compared with other acute leukemia, HAL is reflectory and have worse prognosis. Measuring the immunophenotype of acute leukemia is helpful to the dignosis of HAL.
