摘 要:目的 探讨 β 连环蛋白异常表达和c myc阳性表达在肝门部胆管癌发生、发展过程中的作用及其机理。方法 应用免疫组织化学SP法对 4 2例肝门部胆管癌及 10例胆管良性病变组织中的 β 连环蛋白和c myc蛋白进行检测 ,并结合临床资料进行分析。结果 β 连环蛋白在 10例胆管良性病变组织中均呈正常表达 ,且c myc均呈阴性表达 ;而在 4 2例胆管癌患者中 30例癌组织细胞胞浆内的 β 连环蛋白的异常表达率明显高于胆管良性病变组织 ,且异常表达率与肝门部胆管癌的淋巴结转移显著相关 (P<0 .0 5 ) ,而与肝门胆管癌大小、分化程度和侵袭状况无关 (P>0 .0 5 )。胆管癌中c myc表达阳性率与肝门胆管癌的大小、侵袭状况及淋巴结转移无关 (P>0 .0 5 ) ,而与肝门部胆管癌的分化程度密切相关 (P<0 .0 5 )。β 连环蛋白异常表达与c myc阳性表达在肝门部胆管癌中呈显著的正相关性 (r =0 .32 4 ,P<0 .0 1)。结论 肝门部胆管癌细胞中存在 β 连环蛋白异常表达和c myc蛋白阳性表达的现象 ,与胆管癌的一些生物学行为密切相关。β 连环蛋白的异常表达可能是肝门部胆管癌转移的分子机理之一。Objective To investigate whether abnormal expression of β catenin and high expression of c myc have played a possible role in hilar cholangiocarcinoma carcinogenesis.Methods By using immunohitochemical staining (SP method), the authors detected the expression of β catenin and c myc in 42 paraffin embedded samples of hilar cholangiocarcinoma and 10 benign bile duct disease tissue, and then analyzed the relationship of them with clinical data. Results β catenin was normally expressed in 10 benign bile duct disease tissue, while expression of c myc was negtive. In hilar cholangiocarcinoma tissue, the positive expression rate of β catenin (71.4%) was significantly correlated to the lymphoid node metastasis of hilar cholangiocarcinoma ( χ 2=4.75, P <0.05),but was not statistically correlated to the tumor size,the extent of differentiation and infiltration ( χ 2=3.35,3.45,4.32, P >0.05); the expression rate of c myc (76.2%) was correlated with the extent of differentiation( χ 2=4.87, P <0.05),but not with the size, infiltration, lymphoid metastasis( χ 2= 3.47,4.12,2.76, P >0.05). The abnormal expression of β catenin had relevance to the high expression of c myc with hilar cholangiocarcinoma ( r =0.324, P <0.01). Conclusion The expression of beta catenin and c myc is significantly altered in hilar cholangiocarcinoma, and correlate with biological features of cholangiocarcinoma.The abnormal expression of beta catenin is one of the mechanisms for the spread of hilar cholangiocarcinoma.
