自体TIL治疗恶性胸腔积液的临床疗效观察

摘　　要：目的　探讨自体肿瘤浸润淋巴细胞合并白细胞介素 2 (TIL/IL 2 )过继免疫疗法对恶性胸腔积液的临床疗效。方法　对 2 0 0 2年 6月—2 0 0 4年 3月的 10 7例恶性胸腔积液患者 ,行胸腔积液TIL体外扩增并自体回输 ,观察近期和远期疗效。结果　在可评价疗效的 10 4例患者中 ,70 .2 %完全缓解 (73 /10 4) ,2 2 .1%部分缓解 (2 3 /10 4) ,6.7%无效 (7/10 4) ,总有效率 92 .3 %。完全缓解的病例随访中均未见治疗侧胸腔积液复发。结论　自体TIL/IL 2过继免疫治疗恶性胸腔积液有良好的临床应用价值。Objective To study the clinical effects on malignan pleural effusion treated with self tumor infiltrating lymphocytes combined with interleukin-2(TIL/IL-2). Methods All the 107 patients suffering from malignant pleural effusion were treated with self-transfusion of TIL after amplification in vitro. The immediate and long-term effects were observed.Results The effects in the 104 cases of pleural fluid could be assessed. In the end, 70.2% of them (73/104) were of complete remission (CR), 22.1% (23/104) part remission (PR), and 6.7% (7/104) no remission (NR). The total effective rate was 92.3 %. As for those of CR, no recurrence of hydrothorax was founded during the follow-up.Conclusion The adoptive immunotherapy with self TIL/lL-2 is valuable to the treatment of malignant pleural effusion.
