ICAM-1在胃癌患者腹水血浆中的表达水平及其与TMN分期的关系

摘　　要：目的　探讨ICAM -1在胃癌细胞、胃癌患者腹腔液和血浆中的表达及水平与胃癌TNM分期之间的关系。方法　 3 5例胃癌患者采用流式细胞仪检测肿瘤细胞表面ICAM -1水平 ;用ELISA方法定量检测腹水及血浆中可溶性ICAM -1水平及其细胞阳性表达率。结果　肿瘤细胞表面ICAM -1的表达较对照组明显升高 ,Ⅲ期显著高于Ⅱ期 (P <0 0 5 ) ,淋巴结转移组和无转移组间有显著差异 (P <0 0 5 )。两组Ⅰ期与Ⅱ期间sICAM -1水平无明显差异 (P >0 0 5 ) ,Ⅲ期明显高于Ⅱ期 (P <0 0 5 ) ,有淋巴结转移者明显高于无转移者 (P <0 0 5 )。手术切除肿瘤后 ,血浆中sICAM -1的水平下降 ,并与TMN分期有关。结论　肿瘤细胞表面ICAM -1阳性率及腹水和血浆中可溶性ICAM -1浓度随着胃癌TNM分期增高而有上升趋势 ,且与淋巴结转移有关 ,可作为判断肿瘤发展程度、转移状况以及预后的指标之一。Objective To study the relationship between the ascites, plasma ICAM-1 level and TMN stages in the patients with gastric cancers. Methods 35 gastric cancer patients were enrolled in this study. Flow cytometry and ELISA were used to quantitatively detect the ICAM-1 in gastric cancer ells and sICAM-1 in ascites and plasma of the gastric cancer patients, respectively. Results ICAM-1 expression level in the cancer cells was significantly higher than that in control normal cells,and was much higher in stage III than in stage II(P<0.05). There was significant difference in ICAM-1 level between gastric cancers with and without lymph node metastasis (P<0.05). The sICAM-1 level in ascites and plasma was no difference between stages Ⅰand Ⅱ, but higher in stage Ⅲ and the patients with lymph node metastasis than that in stage II and the patients without one(P<0.05).After sugery, the sICAM-1 level decreased, and the degree of sICAM-1 decrease was correlated with the TNM stage. Conclusion ICAM-1 level in gastric cancer cells and sICAM-1 level in the ascites and plasma of the patients increased with the TNM stage and lymph node metastasis, might be one of indices for evaluating tumor development, metastasis and prognosis.
