艾司洛尔预防气管拔管时心血管副反应的临床观察

摘　　要：目的 :本组为探讨艾司洛尔减轻气管拔管时心血管副反应的效果 ,寻求临床应用的有效方法。方法 :选择 6 0例全麻手术患者 ,ASAⅠ -Ⅱ ,术毕待有拔管指标时拔管。随机分成两组 ,艾司洛尔组 (组Ⅰ ,n =30 ) ,对照组 (组Ⅱ ,n =30 )。采用双盲对照法。组Ⅰ静脉注射艾司洛尔 2mg/kg (加生理盐水 10mL) ,组Ⅱ静脉注射生理盐水 10ml。比较两组用药前后 2min ,拔管即刻 ,拔管后 1min、5min、10min ,SBP、DBP、MAP、HR的监测数据 ,并计算收缩压 -心率积 (RPP)。所有数据以统计学处理。结果 :各组间性别 ,年龄及体重无统计学差异。ECG全部病例均正常。组ⅡBP、HR、RPP全过程变化最小 (P <0 0 1)。有显著差异。结论 :艾司洛尔能预防气管拔管时心血管副反应 ,是临床一种安全有效的方法 ,减轻拔管时心血管不良反应的同时降低心肌耗氧 。Objective:To research the effection of esmolol reducing the untoward reaction of endotracheal outtubation,and explore an effective methoder for clinical application.Methods:60cases general anesthesia patients of ASAI-II,were divided into two groups randomly. while the operation had been finished and outtubation index indicated,group I(esmolol,n=30),group II(control,n=30).The research used the duplexing blind comparison.Esmolol 2mg/kg(0 9%NS10mL)and0 9%NS 10mL were given intracenously to group I and II respectively.Compared the SBP,DBP,MAP,HR data of the two groups at 2minutes before and after outtubation,outtubation instant,and 1minute,5minutes,10minutes after outtubation,then calculated the RPP.ALL of the data were analysed statistically.Results: The gender,age,and weight between the two groups had no significant difference,ECG showed no abnormality.BP,HR,RPP of group II had smaller change at all process than group I,There was significant difference.Conclusion:Esmolol is effective to protect against untoward reaction of circulatory response to endotracheal outtubation.
