摘  要:   目的:探讨喉癌癌变过程中P53、Bcl -2和Bax蛋白表达的临床意义。方法:用免疫组织化学方法,检测20例喉正常黏膜(LNM)、38例喉不典型增生(LAH)和 50例喉鳞癌(LSCC)组织中P53、Bcl- 2和Bax蛋白的表达。结果: LNM、LAH和LSCC组织中P53阳性表达率分别为 0 (0 /20)、52. 6% (20 /38)和 62. 0% (31 /50);Bcl- 2阳性表达率分别为 5. 0% ( 1 /20 )、76. 3% ( 29 /38 )和 50. 0% ( 25 /50 );Bax阳性表达率分别为 75. 0% ( 15 /20 )、50. 0%(19 /38)和 64. 0% (32 /50); 3种组织间P53及Bcl 2阳性表达率差异有统计学意义 (χ2 分别为 22. 722, 26. 698,P均<0. 01),但Bax的表达差异无统计学意义 (χ2 =3. 773,P=0. 152)。LSCC组织中Bcl 2阳性表达率与病理分级和淋巴结转移相关(P<0. 01),P53及Bax与各病理指标均无关 (P>0. 05)。在LSCC组织中,P53和Bcl- 2阳性表达率呈负相关 (rs= -9. 453,P<0. 01 ),而P53与Bax之间、Bcl 2和Bax之间无相关性 (rs分别为 0. 077,-0. 167,P均>0. 05)。结论: P53和Bcl 2异常表达是LSCC发生过程中的早期事件。bcl -2基因过表达与LSCC病理分级和淋巴结转移密切相关。联合检测P53和Bcl 2蛋白的表达状况将有助于LSCC的早期诊断和预后判断。Aim: To investigate the clinical significance of P53,Bcl-2, and Bax expression in laryngeal carcinogenesis. Methods: Immunohistochemical method was used to detect the expression of P53,Bcl-2, and Bax in 20 samples of laryngeal normal mucosa (LNM), 38 samples of laryngeal atypical hyperplasia(LAH), and 50 cases of laryngeal squamous cell carcinoma(LSCC). Results: In LNM,LAH,and LSCC, the positive expression rates of P53 protein were 0(0/20),52.6%(20/38),and 62.0%(31/50),respectively,those of Bcl-2 protein were 5.0%(1/20),76.3%(29/38),and 50.0%(25/50),respectively, and those of Bax protein were 75.0%(15/20), 50.0%(19/38), and 64.0%(32/50).The expressions of P53 and Bcl-2 had significant differences among the 3 kinds of tissues(P<0.01). Bcl-2 positive expression was associated with the pathological grading and lymph node metastasis(P<0.01). In LSCC, a reverse relation was found between P53 and Bcl-2 expression (r s=-0.453,P<0.01)and no relation was found between Bcl-2 and Bax expression or P53 and Bax expression. Conclusion: The abnomal expression of P53 and Bcl-2 can be early molecular events in the carcinogenesis of larynx. Over-expression of Bcl-2 is associated with histological grading and lymph node metastasis. The combined detection of P53 and Bcl-2 protein will be conductive to early diagnosis of LSCC and prognosis.
