鼻咽癌组织中nm23-H_1基因表达与外周血T-AgNORs水平的关系

摘　　要：目的　观察nm2 3 H1 基因产物在鼻咽癌 (NPC)中的表达状态 ,探讨其与NPC的转移及与外周血T淋巴细胞核仁形成区嗜银蛋白 (AgNORs)水平的关系。方法　采用S P免疫组化方法检测6 5例NPC初治患者癌组织中nm2 3 H1 基因产物表达 ,并对治疗前患者外周血T AgNORs含量进行检测。结果　nm2 3 H1 基因蛋白表达率为 6 1.5 % (40 6 5 )。有颈淋巴结转移的患者nm2 3 H1 表达率为5 1.2 % (2 1 4 1) ,低于无颈淋巴结转移患者的 79.2 % (19 2 4 ) ,两者差异有显著性 (χ2 =4 .99,P <0 .0 5 )。随访中有 15例发生远处转移 ,其nm2 3 H1 表达率为 33.3% (5 15 ) ,无转移者为 70 .0 % (35 5 0 ) ,两者差异有显著性 (χ2 =7.5 6 ,P <0 .0 1)。nm2 3 H1 表达水平与T分期无关。nm2 3 H1 基因蛋白表达阳性患者外周血T AgNORs含量显著高于阴性者 (t=5 .5 6 6 ,P <0 .0 0 1)。结论　nm2 3 H1 基因不仅在抑制NPC的转移中有一定的作用 ,而且可能与机体的免疫调节机能有关。Objective To evaluate the role of nm23-H 1 protein expression in nasopharyngeal carcinoma(NPC) and the relationship between nm23-H 1 with the level of AgNORs in the peripheral blood T lymphocyte (T-AgNORs) before radiotherapy (pre-RT).Methods The S-P immunohistochemical method was used to examine the expression of nm23-H 1 in 65 cases of NPC, and the level of T-AgNORs before radiotherapy was determined.Results The overall positive expression of nm23-H 1 was 61.5% (40/65),and nm23-H 1 expression in the NPC with cervical lymph node metastasis and without cervical lymph node metastasis were 51.2% (21/41) and 79.2%(19/24)respectively(χ2=4.99,P<0.05).The positive expression of nm23-H 1 was 33.3%(5/15)in the distant metastasis cases,whereas 70.0%(35/50)in the cases without distant metastasis(χ2=7.56,P<0.01).The level of T-AgNORs was higher in the cases of positive expression of nm23-H 1 than in the nagative cases (t=5.566,P< 0.001).Conclusions The nm23-H 1 gene may play roles not only in inhibition of metastasis of NPC,but also in regulaion of the immunity of the host.
