快速螺钉颌间固定的临床应用研究

摘　　要：目的: 探讨螺钉颌间固定的方法成功率及治疗效果,提供一种对牙齿没有负担的颌间固定器。方 法:研制螺钉颌间固定器,包括3mm×2mm×12mm微型钛螺钉,卡钉螺刀及牵引橡皮圈。38例患者接受治疗,每 例患者均于局麻下,在上下颌前牙及磨牙区两牙根之间,用钻头从牙龈打一导孔,随之用卡钉螺刀将微型钛螺钉垂 直旋入,使之稳固植入牙槽骨内,共植入8个螺钉,然后在螺钉上悬挂橡皮圈,使上下颌处于正常的咬合状态。结 果:38例患者周定14-28d,在固定期间无螺钉松脱现象,均获得了稳定固位,无1例发生感染。经X线摄片及临 床随访观察,全部达到良好的复位固定,未发现咬合错乱及牙齿损伤等并发症。结论: 与传统的颁间钢丝栓结 固定相比较,螺钉颌间固定是一种操作快速,固位牢靠,口腔清洁度好的颌间固定新方法。Objective: To approach a new method of quick intermaxillay fixation. Methods: 8 mini-Ti screws, which were manufactured by ourselves, were rotated into maxillay and mandibular bones from gums. Then rubber-rings were hung on these screws, which make the maxillary and mandibular bones fix into central occlusion relation. Results: Thirty-eight cases used this method in clinic all recovered occlusion relation. During the intermaxillary fixation period, oral cavity was clean well, injury was slight and fracture healed on time. No patient was infected through clinical observation and X ray examination. Conclusion: Screws intermaxillary fixation which have many advantages such as safe, operating simply and quickly, and fixing reliably, is a new ideal inter-maxillary fixation method in our country.
