肝动脉化疗栓塞术在原发性肝癌根治术中的应用价值

摘　　要：目的 :探讨肝动脉化疗栓塞术在原发性肝癌根治术中的应用价值。方法 :对 2 4例原发性肝癌患者 ,男 2 3例 ,女 1例 ,年龄 38～ 6 9岁 ,采用术前介入化疗和栓塞 ,栓塞后 3～ 7日内择期手术 ,术后再行介入化疗和栓塞治疗。结果 :2 4例肿瘤根治术及术前术后 16 8次肝动脉灌注术 (TAI)、肝动脉化学栓塞术 (TACE)均为 10 0 %成功率。复查AFP 19例恢复正常 ,5例有明显下降 ,6～ 2 4个月复发 2 1例 ,经TACE治疗 ,复发灶均有不同程度缩小。死亡 2例 ,中位存活期 19 5月。存活着 2 2例 ,中位存活期已达 2 3 5月。结论 :术前行TACE有利于争取手术机会和手术根治 ,防止术后复发。介入治疗和手术结合是提高肿瘤整体疗效及病人存活率的重要方法。Objective:To explore the using value of transcatheter arterial chemoembolization in radical operation of primary hepatic carcinoma.[WT5”HZ]Methods:Twenty-four cases with primary hepatic carcinoma (23 cases was male and 1 cases female and the age range from 38 to 65 years) were performed with intervent ional chemotherapy and embolic treatment before radical operation. The radical operation was performed 3～7 days after interventional treatment, and then interventional chemiotherapy and embolic treatment were made once more after the radical operation.Results:The success rate of radical operation of 24 cases and 168 times of interventional treatment before and after the radical operation were 100%. Follow-up AFP becomes normal in 19 cases and obvious decrease in 5 cases.Recurrence of primary hepatic carcinoma was found in 21 cases 6～24 mouths after the operation, but the recurrent lesions shrank to some extent after interventional treatment. The median survival time was 19.5 mouths in 2 patients who were dead and 23.5 mouths in the other 22 survival cases.Conclusion:[WT5”BZ]The per-operational interventional treatment is helpful for operational possibility, radical cure and recurrence after the operation. The combination of the interventional treatment and the operation is an important way to increase integral treatment effect and survival rate.
