视神经束测量对脑肿瘤早期诊断价值的探讨

摘  要:应用B超和磁共振对84例脑肿瘤患者进行视神经直径测量,分别计算在不同脑压、不同部位、不同肿瘤性质及不同病程时脑肿瘤患者的视乳头水肿及视神经束增宽的发生率,探讨视神经束增宽对脑肿瘤的早期诊断价值。结果示84例脑瘤患者视乳头水肿的发生率为68%,B超和磁共振测量视神经增宽的发生率为83%和89%(与视乳头水肿相比,P<0.05)。当脑压在1.97~2.50kPa时,肿瘤为胶质瘤,肿瘤位于大脑半球及病程较短时,视神经束增宽的发生率明显高于视乳头水肿的发生率(P<0.05)。结论:B超和磁共振测量视神经束均能提高脑瘤的早期诊断,但B超操作方便,精确性强、经济有效,应列为首选。To explore the value of increasing width of optic nerve in the diagnosis of early stage of brain tumors, the diame-ters of optic nerves of 84 patients with brain tumors were measured with B type ultrasonic tomography and magnetic resonance image (MRI), the rate of increasing width of optic nerve and the rate of papilledema were counted in different conditions of in-tracranial pressure. The results showed that the rates of increasing width of optic nerve measured with B type ultrasonic tomogra-phy or MRI were 83% and 89% , which respectively were higher than that of papilledema (68%, P<0. 05). The results also showed that when the intracranial pressure 1. 97-2. 50 kPa ,being a glioma,the place of tumor being in the cerebral hemishphere and the shout course of the disease, the rate of increasing width of optic nerve were significantly higher comparied with that of papilledema in patients with brain tumors (P<0. 05). The results suggested that measurement of optic nerve with B type ultra-sonic tomography or MRI is helpful to the diagnosis of brain tumors in the early stage, while B type ultrasonic tomography be more useful due to of its convenient operating, high precision, economical and practical.
