摘　　要：目的　观察对恶性脑肿瘤采用超选择脑动脉插管灌注化疗药物的临床疗效和预后。方法　对 32例经组织学确诊和化疗药敏试验的恶性脑肿瘤 ,采用超选择脑动脉灌注卡氮芥 (BCNU)、替尼泊甙 (VM2 6 )、长春新碱 (VCR)、嘧啶亚硝脲 (ACNU) ,同时应用甘露醇、尼膜同 (NIM) ,观察药物反应及近期疗效。结果　 32例经多疗程化疗后 ,1年存活率达 87.5 % (2 8/ 32 ) ,2年存活率为 6 8.7% (2 2 / 32 ) ,全部患者均无眼部症状及严重的神经毒性反应。结论　应用超选择脑动脉化疗法治疗恶性脑肿瘤 ,其患者存活率较单纯颈内动脉灌注化疗及外周静脉化疗高、并发症少 ;体外化疗药物敏感试验可助于化疗方案的合理制定 ,提高颅内恶性肿瘤患者的远期存活率。Objectives To evaluate the clinical efficacy and prognosis of superselective intra-arterial chemothapy for the treatmant of malignant intracranial tumorsMethods 32 cases of malignant intracranial tumors were treated by superselective intra-arterial infusion BCNU,ACNU,VM26,VCR and together with Mannitol,Nimotop after the pathological diagnosis and chemotherapy chemosensitivity test in vitro were establishedEarly efficacy of chemotherapy was assessedResults 32 cases of the patients were treated by superselective intra-arterial infusion for different period of treatment1 year survival rate 875%(28/32),2 years survival rate 687% ( 22/32)No symptom of neurotoxicity and vision decrease was displayed in those patientsConclusions The method is helpful in prolonging survival period than that in transcarotid artery and venous injection,with little side effectsChemotherapy under the guidance of chemosensitivity in vitro may establish reasonable methods and improve the long time survival rate of patients with intracranial malignant tumors
