摘 要:目的:早产儿的智力发育水平能否赶上足月儿一直是家长和儿童保健专家关注的焦点。观察早产儿婴儿期的智力发育水平,分析胎龄和出生体质量与早产儿智力发育水平的相关关系。方法:以2000-05/2002-07在济南市中心医院出生且在济南市区居住的早产儿(胎龄31~36周)、低出生体质量儿(出生体质量1750~2450g)62例及同期出生的足月、顺产新生儿92例为研究对象。采用Gesell格塞尔发育诊断量表(Geselldevelopmentdiagnosisscale,GDDS)和婴儿~初中学生社会生活能力量表即日本S-M社会生活能力检查修订版对两组婴儿的智力发育水平进行评定。结果:早产儿GDDS各能区发育商值和日本S-M社会生活能力评定值明显低于足月儿(P<0.05),且与胎龄和出生体质量呈正相关关系。GDDS适应性发育商:Y=-85.5728+5.0444X(X为胎龄),r=0.7205,GDDS适应性发育商:Y=43.2587+21.4588X(X为出生体质量),r=0.7791,日本S~M社会生活能力Y=-6.4498+0.4491X(X为胎龄),r=0.665,日本S-M社会生活能力Y=5.0988+1.8736X(X为出生体质量),r=0.7052。结论:早产儿婴儿期智能发育水平明显落后于足月儿。AIM:Whether the level of intellectual development in premature infants can equal that in term infants has always been attracting the attention of the parents and experts in child health care.The aim of this study is to investigate the status of intellectual development of premature infants in infancy,and analyze the correlation of gestational age and birth body mass with the status of premature infants' intellectual development. METHODS:The study subjects were 62 infants including premature infants(gestational age:31 to 36 weeks) born in Jinan Central Hospital between May 2000 to July 2002 living in Jinan and those with low birth body mass(1 750 to 2 450 g),and 92 mature term infants born spontaneously at the same periods.The Gesell development diagnosis scale(GDDS) and social life ability scale for children from the infants to middle school students,that was Japanese S-M social life ability scale for intellectual development(revised edition) were used to assess the intellectual development of all the subjects. RESULTS:The development quotient(DQ) in intelligence fields of GDDS and the assessed value of Japanese S-M social life ability scale in premature infants were significantly lower than those in term infants(P< 0.05),and positively correlated with gestational age and birth body mass.Adaptable DQ of GDDS was Y=-85.572 8+5.044 4X(X stands for gestational age),r=0.720 5,and adaptable DQ of GDDS was Y=43.258 7+21.458 8X(X stands for birth body mass),r=0.779 1.Japanese S-M social life ability was Y=-6.449 8+0.449 1X(X stands for gestational age),r=0.665,and Japanese S-M social life ability was Y=5.098 8+1.873 6X(X stands for birth body mass),r=0.705 2. CONCLUSION:The level of intellectual development of premature infants in infancy is significantly lower than that of term infants.
