两种桩核系统修复上前牙残冠的临床研究

摘　　要：目的　比较圆柱形预成桩核和锥形铸造桩核修复上前牙残冠的临床效果。方法　收集上前牙残冠患者共 15 9例 ,随机分两组 ,分别进行圆柱形预成桩核 (预成组 ,38例 )或锥形铸造桩核修复 (铸造组 ,12 1例 ) ,两组均以金属烤瓷冠修复。临床观察 :修复体脱落和 (或 )基牙根折情况 ;基牙龋损情况 ;牙龈炎。结果　随诊 3年 ,预成组38例 4 5件中脱落 1件 (2 % )、无根折 (0 % )、基牙龋损 2件 (4% )、牙龈炎 2件 (4% ) ;铸造组 12 1例 15 3件中脱落 6件(4% )、根折 4件 (3% )、基牙龋损 4件 (3% )、牙龈炎 4件 (3% )。两组间各并发症的发生率无显著性差异。结论　预成桩核的使用可减少就诊次数 ,操作简便、使用安全 ,值得临床上推广应用。Objective To compare the clinical effect of cylinder prefabricated post and core (CPPC) with conical cast Post and Core (CCPC) on restoration of severe defective crown of upper Anterior teeth. Methods 159 patients with severe defective crown of upper anterior teeth were included from February 1999 to June 2001 and divided into two groups randomly: one group was restored by CPPC and porcelain-fused-to-metal (prefabricated group, 38 patients with 45 teeth) and the other group restored by CCPC and porcelain-fused-to-metal (cast group, 121 patients with 153 teeth). The clinical observation following-up for 3 years was included: luxation of denture, root fracture, secondary caries and secondary gingivitis. Results In the 45 teeth of prefabricated group two times operation were needed and there were one denture of luxation (2%), no root fracture, two teeth of secondary caries (4%) and two teeth of secondary gingivitis (4%). In the 153 teeth of cast group three times operation were needed and there were six denture of luxation (4%), four teeth of root fracture, four teeth of secondary caries (3%) and four teeth of secondary gingivitis (3%). No significant differences of complications were observed between the two groups (P>0.05). Conclusion The operation times can be shortened by using of prefabricated post and core. CPPC was operated simply and safely and worth to recommend in clinical application.
