摘　　要：目的:探讨骨骼肌肉系统肿瘤和肿瘤样病变周边MR灌注成像特征,以进一步提高MR灌注成像在骨骼肌 肉系统肿瘤鉴别诊断中的价值。方法:对2002年4月～2004年2月41例骨骼肌肉系统肿瘤进行MR灌注成像。计算肿 瘤周边与肿瘤周围动脉时间 信号强度曲线(SI T)的最大线性斜率(Smax)及三者之间的强化峰值差异(ΔSI)、最大强化时 间差异(Δt)和强化差异率(R)。结果:骨骼肌肉系统良、恶性肿瘤Smax和ΔSI差异无显著性意义(P>0.05);良、恶性肿瘤 Δt的均值分别为27.5s和23.4s,差异存在显著性意义(F=4.63,P<0.05);良、恶性肿瘤R值的均值为-0.15和0.62, 两者差异存在显著性意义(F=2.39,P<0.05)。结论:Δt和R值在骨骼肌肉系统良、恶性肿瘤之间有显著性差异,具有一 定的鉴别诊断意义。Objective:To compare the characteristics of rims and peripheral arteries of benign and malignant musculoskeletal tumors and tumor-like lesions on perfusion MRI.Methods:Perfusion MRI was performed in 41 cases of benign and malignant musculoskeletal tumors and tumor-like lesions.The steepest slope of the SI-T curve of the rim of neoplasms (S max ),rim-to-artery enhance-peak difference (ΔSI),enhance-peak-time difference (Δt) and different perfusion ratio (R) of benign and malignant musculoskeletal tumors and tumor-like lesions were recorded and compared.Results:The difference in either S max or ΔSI between malignant and benign musculoskeletal neoplasms had no statistically significance (P>0.05); the median of Δt in benign neoplasms is 27.5s,which is compared with 23.4s in malignant ones;Δt of benign and malignant neoplasms differed significantly (F=4.63,P<0.05);the median of R is -0.15 and 0.62 in benign and malignant neoplasms and differed significantly in two series (F=2.39,P<0.05).Conclusion:Benign and malignant musculoskeletal tumors and tumor-like lesions differed significantly in △t and R value and insignificantly in other indexes.
