甲胎蛋白mRNA与原发性肝癌细胞血液播散的相关性研究

摘　　要：目的 :探讨AFPmRNA与原发性肝细胞癌 (PHC)血液播散的关系及其临床意义。方法 :用反转录套式基因扩增技术检测了 16 8例不同病期PHC患者静脉血有核细胞成分中的AFPmRNA ,以不同肝病、消化道肿瘤和正常人作对照 ,并分析了AFPmRNA检出率与临床特征的关系。结果 :AFPmRAN检出率在肝外转移者、癌栓形成者和肝内转移者均显著高于无转移者 (P <0 .0 1) ;肿瘤直径 >5cm者AFPmRNA检出率显著高于肿瘤直径 <5cm者 (P <0 .0 1) ;术后AFPmRNA持续阳性者易发生复发或转移。结论 :动态检测AFPmRNA有助于早期发现PHC血液播散、术后复发及判断预后。Objective:To inquire about the significance of AFP gene express ion for diagnoses the hematogenous spreading of primary hepatocellular carcinom a(PHC).Method:AFP mRNA in peripheral blood of 168 patients with PHC and control subjects were detected by reverse transcriptases nested polymerase chain reaction,and the relation between the detection rate of AFP mRNA a nd the clinical parameters were analyzed. Results:AFP mRNA positive rate in cases of PHC with extrahep atic metastases,portal vein thrombosis and intrahepatic metastases are markedl y higher than that of control subjects(P<0.01).Among the patients with PHC, th e size of tumor is markedly increased in patients with AFP mRNA in blood.T he cases which showed AFP mRNA(+) after resection are commonly occurence or metastases.Conclusion:AFP mRNA may be an indicator of circulating PHC cells,which might predict the hematogenous spreading ,recurrence and prog nosis in PHC.
