摘  要:目的 探讨血管内皮细胞生长因子(VEGF)和血管生成与多发性骨髓瘤(MM)发展的关系。方法 采用EnVision免疫组织化学二步法检测 20例MM患者骨髓中VEGF蛋白的表达及微血管数,比较治疗前、后骨髓VEGF蛋白表达和微血管数的差异,分析其与患者部分实验室检查指标及临床疗效的相关性。结果 化疗后部分显效患者VEGF蛋白的表达及骨髓微血管数显著低于初发未治患者 (P<0. 05),而在化疗后微小显效患者中无显著下降(P=0. 446和 0. 950)。初发未治、部分显效和微小显效患者VEGF蛋白的表达及骨髓微血管数均显著高于对照组(P<0. 01)。结论 VEGF可能是骨髓瘤细胞的一种自分泌因子,同时可作为一种旁分泌因子调控患者骨髓中的血管新生反应。VEGF可能是MM患者血管生成反应的主要调控因子之一。Objective To investigate the relationship of vascular endothelial growth factor and angiogenesis in the progression of multiple myeloma (MM). Methods The role of bone marrow angiogenesis among different stages of multiple myeloma in 20 cases was elucidated by using EnVision two-step immunohistochemical staining method. The differences of bone marrow microvessel counts between different disease stages, and its relationship with some laboratory variables and the response to chemotherapy were also analysed. Results The expression of VEGF protein and bone marrow microvessel counts in patients with partial chemotherapeutic response were significantly lower than those in the untreated newly diagnosed patients (P<0.05), however insignificant decrease was shown in patients with minimal chemotherapentic response (P=0.446, 0.950). The expression of VEGF protein and bone marrow microvessel counts in partial response or minimal response newly diagnosed patients were all significantly higher than those in the contral group(P<0.01). Conclusion VEGF may be an autocrine factor, concomitantly it modulates the angiogenic reaction in the bone marrow as a paracrine factor. VEGF may be one of the major angiogenic modulators in multiple myeloma patients.
