摘 要:目的 探究锁孔手术治疗颅底和脑深部肿瘤的手术技巧和适应症。 方法 对 3 0例颅底和脑深部肿瘤行锁孔显微手术治疗 ,其中部分用脑内窥镜及超声辅助。 结果 2 6例实质性肿瘤中 2 3例全切除 ,3例次全切除 ,其它 4例囊性肿瘤全切除 ,无手术死亡、致残和并发症。 结论 锁孔显微手术是治疗颅底和脑深部肿瘤的微创技术 ,适当选择适应症和设计手术方案尤其重要 ,辅以脑内窥镜及超声的应用可提高手术疗效。Objective To study the operation techniques and indications of keyhole microneurosurgery used for deep-seated tumors in the brain and at the skull base. Methods 30 cases of deep-seated tumors were treated by microsurgery with keyhole craniotomy. Some of them were treated with endoscope-assisted microsurgery and some with intra-operative ultrasound scan. Results 23/26 cases solid tumors were totally removed and 3/26 cases were subtotally removed. 4 cases cystic tumors all were totally removed. There were no morbidity, mortality or complication related to craniotomy. Conclusion Keyhole microneurosurgery is a strategy of minimally invasive techniques to deal with the tumors deep-seated in the brain and at the skull base. However, it is important to select proper cases with correct indications and individually design on the treatment strategy. Neuro-endoscope and intra-operative ultrasound scan may improve operation outcome.
