摘　　要：目的　探讨 HIFU治疗子宫肌瘤的方法 ,了解其疗效。对超声在术中定位、监控与术后疗效判断中的应用进行评价。方法　对 15 3个子宫肌瘤行 HIFU治疗。治疗采取由深至浅逐层定位的方法。治疗时膀胱须充盈至全部覆盖靶目标。术中应用 B超定位与监控 ,术前定位及术后随访应用 CDFI。结果　治疗后肌瘤缩小 ,血流信号减少或消失。回声变化比较复杂 :可有回声增强、强弱不均匀、无明显变化三种表现。临床症状改善。结论　HIFU治疗子宫肌瘤只要方法得当 ,会取得满意的疗效。超声定位精确 ,可实时监控治疗 ,并能准确、客观地评价疗效。Objective To investigate the method of treating hysteromyoma with high intensity focused ultrasound and to evaluate the value of ultrasound in intraoperative localization,mornitoring and judging the treatment effect of hysteromyoma.Methods Totally 153 hysterormyomas were treated with high intensity focused ultrasound.During treating,locating was conducted one after another from the deep tier to the shallow tier,and the bladder had to be full enough to cover the target zone.Ultrasound was used to locate and mornitor during the treatment.CDFI was used to locate before treatment and to reexamine after treatment.Results After treatment,hysteromyomas shrank.Blood supply was reduced or disappeared.The changes of echoes varied:some became enhanced,some heterogeneous,and others without obvious change.The clinical symptoms were improved.Conclusions Treating hysteromyoma with HIFU can creat satisfactory clinical effect when methods are used properly.Ultrasound can help to locate precisely,to monitor the treating process and evaluate the clinical effect objectively and accurately.
