摘 要:目的 研究肺癌患者外周血中CEA mRNA、CK19 mRNA的表达及其联合检测的临床意义。方法 采用巢式PCR技术检测 4 3例肺癌患者、12例肺良性疾病患者和 13例健康人外周血CEA mRNA、CK19 mRNA的表达 ,结合肺癌患者常规病理检查、肿瘤类型、临床分期对检测结果的敏感性、特异性进行统计学分析。结果 4 3例肺癌外周血标本中CEA mRNA阳性 2 1例 (48.8% ) ,CK19 mRNA阳性 2 4例 (5 5 .8% ) ;12例肺良性病变中CK19 mRNA阳性 1例 (8.3% ) ,CEA mRNA均为阴性 ;13例健康人外周血中CEA mRNA、CK19 mRNA的表达均为阴性。在小细胞肺癌、腺癌、鳞癌中 ,CEA mRNA的表达阳性率分别为 16 .7%、81.3%、33.3% ,CK19 mRNA的表达阳性率分别为 33.3%、5 0 .0 %、6 6 .7%。Ⅲ、Ⅳ期肺癌CEA mRNA和CK19 mRNA的表达阳性率高于Ⅰ Ⅱ期。结论 CEA mRNA、CK19 mRNA可作为标志物来检测肺癌患者外周血微转移 ,联合检测有助于提高准确性。Objective To study the expression of CEA-mRNA and CK19-mRNA , and the clinical significance of combined examination of CEA-mRNA and CK19-mRNA in peripheral blood of patients with lung cancer.Methods Using nested PCR, we examined the expressions of CEA-mRNA and CK19-mRNA in the peripheral blood of 43 patients with lung cancer, 12 patients with benign diseases, and 13 healthy controls.In the patients with lung cancer,statistical analysis was made of the pathological types of tumors,the clinical stages and the sensitivity and specifity of assays.Results Among the 43 patients with lung cancer , there were 21 patients with positive expression of CEA-mRNA( 48.8%), and 24 patients with positive expression of CK19-mRNA(55.8%).Among the 12 patients with benign diseases, there was only one patient with positive expression of CK19-mRNA(8.3%), and the expression of CEA-mRNA was negative for all the 12 patients.For the 13 healthy controls,the expressions of CK19-mRNA and CEA-mRNA were all negative.In patients with small cell carcinoma, adenocarcinoma, and squamous carcinoma of lungs the rates of positive expression of CEA-mRNA were 16.7%, 81.3%,and 33.3%,respectively; and those of CK19-mRNA were 33.3%,50.0%,and 66.7%,respectively.The rates of positive expression of CEA-mRNA and CK19-mRNA in stages Ⅲ and Ⅳ were higer than those in stages Ⅰ and Ⅱ.Conclusion Both CEA-mRNA and CK19-mRNA can be used as markers to detect the micro-metastasis in the peripheral blood of patients with lung cancer; therefore,combined assay can improve the accuracy.
