腔镜下甲状腺叶部分切除术的体会

摘　　要：目的　探索利用腔镜行甲状腺部分切除的方法及适应证。方法　用腔镜为 64例甲状腺腺瘤、甲状腺囊肿,结节性甲状腺肿,甲亢及甲状腺癌的病人实施了经胸入路甲状腺部分切除术。结果　全部病例手术成功,前 38例平均手术时间 160min.后 26例平均手术时间 90min。暂时性声嘶 2例,暂时性呛咳 2例,皮下积液 1例。未发生术后出血,无中转开放手术。术后恢复良好,近期随访颈部皮肤感觉接近正常。结论　腔镜行甲状腺部分切除术治疗良性甲状腺病变具有无颈部疤痕,美容效果好,皮肤感觉接近正常,恢复快等优点。对适应证明确者,是值得推崇的手术方法。Objective To evaluate the clinical application of endoscopic partial-thyroidectomy.Methods Partial-thyroidectomy with endoscopic instruments throuth chest-wall approach was done for 64 patients with adenoma, cyst, multipule nodes and cancer of thyroid.Results All 64 cases of operation were successful with no conversion. There were no severe surgical complications such as nerve damage or hemorrhage. Everage operation duration was 160min for the former 38 cases and 90 min for the current 26 cases. The mean time of hospilization after operation was 4 days.Conclusion with the advantages of minimal and hidden incision, nearly normal feeling of neck skin and sooner recovery, the endoscopic partial-thyroidectomy will previal in selected cases in the future.
